Job Description

Title: Brain Injury Care Co-ordinator

Responsible to: Services Manager

Main Purpose: To provide comprehensive support to brain injury survivors.

Hours: Initially 2 days per week (15 hours) increasing as the service develops

Location: Central Milton Keynes

Annual Leave: 25 days per annum (pro rata) excluding bank holidays

Key Tasks: Provide physical, cognitive, social and emotional rehabilitation to Headway clients, their families and help them to improve life after brain injury.

Headway’s client group consists of people living with an acquired a brain injury.

The wide-ranging effects of a brain injury can drastically affect not only the life of the client but also that the close family. These are an additional client group who also need information, advice and support.

As a Care Co-ordinator, Headway Milton Keynes will expect you to work with the clients but also all other family members and outside agencies involved in their life. You will be part of a team who aim to assist in providing physical, cognitive, social and emotional rehabilitation to all referred clients and their families, in an attempt to help them regain a meaningful life.

You will report to the Services Manager.

Duties and responsibilities

1.  Plan and run group cognitive sessions

·  Briefing and organising volunteers before each group session

·  Collection of debrief notes on each client from volunteers

·  Filing clients’ work in their respective folders

·  Oversee lunch when appropriate

·  Awareness of clients’ whereabouts during sessions

2.  Assist clients during one to one sessions

·  Working directly with clients

·  Providing effective debrief notes for clients

·  Filing client’s work

3.  Key Working

·  Maintain effective communication between Headway and client

·  Continually appraise and update individuals’ care plans

·  Monitor client goals on a monthly basis

·  Attend care plan reviews and provide feedback on clients’ progress

·  Liase with any outside agencies involved in clients’ care

·  Be a point of contact for outside agencies

·  Plan individual folders where appropriate

·  Source alternative activities outside of Headway

4.  Administration

·  Update database following each session

·  Update clients’ folders

·  Plan and organise care plan reviews for key clients

·  Type and file care plan reviews for key clients

·  Organise, plan and attend day trips for enjoyment and enhancement of social skills

·  Update attendance records

5.  General Housekeeping

·  Assist with general washing up when required

·  Assist with locking up procedure

·  Ensure Headway Milton Keynes is generally neat and tidy

6.  Other

·  To comply with Headway’s Equal Opportunities and Health and Safety policies and undertake appropriate training and personal development as appropriate

·  To undertake such other reasonable tasks as may, from time to time, be deemed necessary.
