JOB DESCRIPTION
Job Title:Family Planning Health Care Provider
Department / Program:Jasper Office
FLSA:Non-Exempt

GENERAL DESCRIPTION
Provides health care for women in the Jasper Office under the direction of the medical director. Participates in examining, diagnosing, counseling and medical treatment as indicated per protocol.

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING, OTHER RELATED DUTIES MAY BE ASSIGNED:
(10%)Administer immunizations
(25%)Perform physical exams for Family Planning and Breast and Cervical Cancer
Services
( 5%)Review records and laboratory results
(10%)Referral/Follow-up on clients referred to in-house and outside providers
(20%)Counseling and Treatment of clients per established protocols
Telephone contacts with clients incoming and outgoing
Dispense Birth Control Pills
(25%)Responsible for proper chart documentation
( 5%)Supervise clinic staff

GENERAL QUALIFICATIONS
MINIMUM QUALIFICATIONS
Education and Experience
Nurse Practitioner
Graduation from an accredited four-year college or university with major course work in nursing.
Graduation from an accredited Nurse Practitioner Program
Experience in nursing work; preferably a public health clinic setting with some supervisory responsibilities
Experience and education may be substituted for one another.
Physician’s Assistant
Graduation from an accredited four year college or university with major course
in health care.
Graduation from an accredited Physician Assistant Program.
Experience in Physician Assistant, preferably a public health clinic setting
Experience and education may be substituted for one another.
Medical Doctor or Doctor of Osteopath
License to practice medicine.
Knowledge, Skills and Abilities
Professional nursing theory, nursing processes, nursing sciences, and clinical skills
Principles and practice of public health nursing and health care management
Skills in:
Interpreting laboratory findings, physical signs/symptoms, and information on medical charts
Developing a plan of action for resolving problems identified

Ability to:
Communicate effectively both verbally and in writing
Organize and prioritize workload and meet deadlines
Establish and maintain effective working relationships

Must have reliable transportation (when JNCPHD vehicle unavailable)

Licensed to practice medicine in the State of Texas
Physical Condition
Good health and physical condition sufficient to permit the full performance of the duties of the position.
