Janice S. Hanson Award for Outstanding Healthy Community InitiativesNomination Form

Criteria: The Janice S. Hanson Award for Outstanding Healthy Communities Initiatives was established in 2014 in honor of Jan’s 12 years of service and dedication as the Coordinator of CHNA15.This award will be given to a CHNA Grantee who has demonstrated a commitment to healthy community principles and has brought forward, through his or her work, opportunities for furthering the goals of the CHNA and the Community Health goals of DPH. The Award is given to continue the work for which the recipient has received CHNA funding. The Janice S. Hanson Award is a monetary award,to be determined by the CHNA15 Steering Committee,to continue the work for which the recipient has been recognized. The recipient will be announced at the CHNA 15 2018 Annual Meeting.

Deadline: Nominations for the 2018 Janice S. Hanson Award must be received by April 4, 2018, and should be emailed to . The CHNA 15 Steering Committee will select the award recipient.

Eligibility: Nominees must have received a grant from CHNA 15 within the past 3 years. Nominators must be members in good standing of CHNA 15.Member:An individual who attends at least two CHNA meetings within the previous twelve months. The meetings attended may be General Meetings, Annual Meeting, Steering Committee Meetings or any scheduled Ad Hoc committee meetings.Previous recipients of the Janice S. Hanson Award are noteligible. You may not nominate yourself.
