JABSOM Annual Faculty and Chair Report Procedures

Purpose
The JABSOM Annual Faculty and Annual Chair Reports are for the purpose of providing annual feedback to each faculty member and department chair at the end of each academic year. The Annual Report is an online self report that serves as a snapshot of each faculty and chair’s academic goals, time allocation, and productivity information in the areas of teaching, research, service, and clinical (as applicable) for each academic year. Reporting period is for each academic year (July 1 – June 30).
There is also a survey for faculty to complete to rate the quality of their Department Chair’s leadership skills. These surveys will be tallied and an overall score in each area evaluated will be submitted to the Dean and will remain anonymous. This is to provide the Dean with annual feedback about each chair. The Dean may then use this feedback during annual meetings with each chair.
Process
Faculty and chairs will complete a report via the secured online reporting system which can be found on the Faculty Development Program website under Annual Faculty Reportshttp://jabsom.hawaii.edu/faculty/facdev/annual-faculty-reports/
Log-in to the online system will require a UH email username and password.

If you have completed a report in the past, your data will be stored in the system and ready for updating each year.
The Annual Reports will provide feedback that may be used to draft the narrative assessments that are to be submitted with the Annual Evaluation and Contract Renewal form (form Attachment A or form Attachment B) that must be completed for faculty by December 31 of each year. The Annual Reports should also serve as an annual feedback tool for preparing faculty to apply for promotion and tenure.
For Faculty:
Faculty members will receive feedback from their department chair regarding their productivity and strengths and weaknesses during a face-to-face meeting, at which time goals and plans to overcome any weaknesses will be developed for the next year.
Goals and a plan to help the faculty member prepare to apply for promotion and/or tenure should also be documented at this time. Chairs should document strengths and weaknesses in the last section of the faculty member’s report online. Faculty member and Chair should both sign a copy of the last page of the report at time of meeting and the department should keep it on file as a record of the meeting.
For Chairs:
(The same process above will apply to Department Chairs, except that Chairs will be reviewed by the Dean.)
Timeline/Deadlines
Every effort will be made to have reports ready for the current academic year available online by April 30. However, every year the reporting system goes through a series of upgrades to make it more user-friendly, therefore in the event that the reports are not available by April 30, deadlines will be pushed back and all faculty and chairs will be notified of the revised submission deadlines.
Submission deadline for faculty is: June 30 (midnight) (or as otherwise noted)
Submission deadline for Chairs is: July 31 (midnight) (or as otherwise noted)
Reports will close on July 1 for faculty and August 1 for Chairs. (or as otherwise noted)
Individual meetings with each faculty member and department chair will take place between July 1-December 31 of each year. Chairs will schedule meetings with faculty and the Dean will schedule meetings with Chairs.
Reports will close on December 31 for chair review and entering of feedback into the online system.
