I am writing as Investigating Managerto inform you that you are requiredto attend an investigatory meeting relating to allegations that:

As part of the investigation process, please could you submit a written statement which must be signed and dated, pleasebring this with you to the meeting or ensure it is provided to me by (insert date).
This is a formal meeting during which notes will be taken, however this meeting is not a disciplinary hearing.
You will receive a letter within 14 days of the completion of the investigation to advise you of next steps. This could include:
- An invitation to a meeting to discuss the outcome of the investigation
or
- An invitation to a formal disciplinary hearing.
\you are advised that should a formal disciplinary hearing be calledthe notes of this meeting, your written statement and any information that comes to light during the course of the investigation may be used at the hearing.If this is the case, prior notification will be given in accordance with the organisations Disciplinary Procedure, a copy of which is enclosed.
In order that the investigation can be conducted objectively you must not discuss the incidents with colleagues that were on duty at the time or those who may be involved with the investigation. If you are unsure who you may discuss this with please do not hesitate to contact me or take advice from the HR Consultancy Team or your staff-side representative.
If you have any special requirements that we need to consider for the meeting, please advise in advance e.g. Disability/Access.
I would remind you that all matters relating to this investigation must be treated in confidence.
