EMPLOYEE CHARITY OF CHOICE – APPLICATION QUESTIONNAIRE

OVERVIEW

The Employee Charity of Choice is a program of Manitoba Liquor & Lotteries – The Together Project™ that supports capital projects or programs that benefit children and youth in Manitoba.

Application Deadline:January 17, 2018 at 12 noon

Please attach all necessary documentation with your submission:

  1. Application Questionnaire (this document)
  2. Audited Financial Statements (preferred) or Unaudited Financial Statements and a recent Bank Statement
  3. Project Quotes, where applicable (min. two preferred)

Note: Prepare/complete all the documents noted above and scan/save them to your computer prior to completing your online submission. You will need to upload these documents with your submission. Incomplete applications may be disqualified.

FUNDING CATEGORIES

Note: Category A funding will be awarded over a two-year period, while Category B and C funding will be dependent on the program/project selected and may be awarded in a lump sum payment upon signing of agreement, with a portion withheld until after completion.

Registered charitable and non-profit organizations are encouraged to apply for funding fora Capital Project or New Program as outlined below:

  1. Capital Project
  2. A new construction, expansion, renovation or replacement project for an existing facility or facilities. Project costs mayinclude the cost of land, engineering, architectural planning and contract services needed to complete the project (e.g. community club, skating rink, swimming pool and additions or upgrades to an existing facility).
  3. The purchase of major equipmentwith a useful life of at least five years (e.g. ice re-surfacer, commercial freezer/stove/refrigerator).
  4. A major maintenance or rehabilitation project for existing facilities with a useful life of at least five years(e.g. a new roof for a building, flooring in a community centre or repair of water damage to a facility).
  5. New Program
  6. New program
  7. Existing program that is being expanded to new area(s)

Program costs may include equipment and supplies; operating costs, such as salaries and rent for the new/expanded program may be considered and will be assessed relative to the program. Existing organization/program costs are not eligible for funding (e.g. rent, utilities, etc. at existing facility).

ELIGIBILITY CRITERIA

Must be a registered non-profit or charitable organization:

  • for a minimum of five (5) years to qualify for Category A (Province-wide Project/Program) funding; or
  • for a minimum of three (3) years to qualify for Category B & C (Local Project/Program).

Must be based in Manitoba or have a Manitoba Division.

Must benefit children and/or youth (Ages 0 to 29) in Manitoba.

Must be broad-based and accessible to the general public.

TOTAL project value falls within one of the Categories outlined above.

Manitoba Liquor & Lotteries is the exclusive funder of the program/project (i.e. requests for partial funding are ineligible).

Must be able to demonstrate sound financial practices and have a plan to sustain the program/project into the future.

OTHER INFORMATION

  • If successful, the organization must provide progress reports up to two times annually, or as outlined in the signed agreement. Payments may be withheld until progress reports are received and satisfied.
  • If more than one proposal is submitted by an organization and selected, only one program/project is eligible for funding. The organization will be contacted to decide which program/project is their preferred proposal to be considered.
