Dear Bill:

This letter has two purposes: I would like to let you know that I am formally submitting a complaint, and I am using this letter to serve as my initial documentation.

The substance of my complaint is that on or about the week of September 16th, 2013, when I returned from my medical absence, I felt that I was being discriminated against due to my psychiatric disability due to the time I had to take off for my mental stability. I feel that my employment is in jeopardy because of my disability because of the time it causes me to be home from work, and suspect that interviews were being held the week of September 16, 2013, for the position I hold, based on the job advertisement that was posted August 6, 2013, after a different absence I had related to my condition. I must note that on May 7, 2013, I verbally asked my supervisor, Janet Collins, if I could be allowed to work from home during such times of distress, and I was told no.

In addition, I suspect that some of the details of my medical condition and reasons for being absent were shared with employees outside of the human resource department during the time I was out of the office, September 5, 2013 to September 16, 2013. That is the substance of my complaint.

My initial reaction was to discuss the situation with Janet Collins on the afternoon of September 20, 2013 after working hours for more privacy, but upon re-entering the building at approximately 5:15 pm, I came across Janet with a woman that was not an employee of PMi. As I approached them, it is my belief that Janet conveyed to the unidentified woman that it was my job she was interviewing for. This had a profound effect on me, which led me to your office immediately after. Unfortunately, I was in emotional distress internally that I could not bring myself to discuss these incidents with you at that time. After investing a great deal of thought I have decided to present you with this information in writing.

I truly believe that it is not PMi’s intent or policy to permit such instances to occur without a review of the circumstances. My hope is that the situation will be resolved and I will be able to continue my employment with PMi without any type of retaliation for this complaint or further discrimination.

I appreciate your review of this matter. I have always enjoyed my work responsibilities at PMi, and I hope you will recognize my attempts to rectify a situation that, to me, is unbearable.
