For reviews involving cross-Faculty/College collaboration, please ensure that the Lead Faculty/College with administrative responsibility for the course(s) is identified.
2 / For proposals involving collaborative partners please list the name of the collaborative partner(s)
3 / Identify Chair, External Academic (post, place of work), internal member(s), Faculty Quality Manager/Head of Academic Services and Reporting Officer
4 / Identify Faculty/College, if other than Lead Faculty/College
5 / Identify other attendees and their designations, for example, members of the senior management team or students
