Price Agreement #UN09-061

Addendum #2 - Agreement Extension

This agreement is hereby extended from July 1, 2014 through June 30, 2019. The IUC-PG reserves the right to extend the resultant Price Agreement beyond the normal expiration date if the IUC-PG determines it to be in their best interest and JPMorgan Chase agrees to the extension. The attached Second Amendment to Commercial Card Agreement is hereby made a part of this Agreement Extension. All other terms and conditions of the original agreement remain in effect and are made a part of this Agreement Extension.

Members interested in signing up for this program will be asked to sign the Participation Agreement, shown as Exhibit C on pages 24–25 of the Commercial Card Agreement and may need to sign the Single Use Account Services Addendum, if applicable. Contact the following individual to begin:
