1. / Apologies and IntroductionsApologies were noted.
Guy opened the meeting and advised that the name of the Connecting People Forum had been changed to Integrated Health, Housing & Social Care (IHHSC) to reflect the broadening scope and remit of the group.
Guy advised of two new members who will be attending the Forum from December onwards:

· Dawn Thornber from Contour Homes
2. / Minutes and Matters Arising from previous meeting held on 18 June 2015
The following matters were discussed:
Work Programme – Action: Jane will circulate the performance report as soon as she receives approval to do so.
Alcohol Services Review– at the moment there are no changes to the specialist alcohol services. Action: Jane to advise further when she receives an update about the final intentions.
Letter to MCC: How organisations review/develop capital funding opportunities linked to housing care and support grant and other capital pots – Guy has still not received any response. Action: Rob to follow up.
Purple Futures – workshop planned to look at the rehousing of ex offenders and Manchester Move. No update as yet. Actions: (a) Guy to speak to Mike Stephens (b) Jane to establish how commissioning payments will work in the future between SP funded services where ex offenders being rehoused. (c) Jane to liaise with Sonia Andrade about setting up a workshop between housing and public health re proposed key worker model.
Older People’s Provider Networking Group – Action: Minutes to be circulated with the IHHSC papers – Zoe Robertson, Lead Commissioner.
3. / Housing Related Support Programme Update
Jane updated members advising that contract extension/variation letters had been sent out to all mental health and socially excluded HRS Providers. In the main, contracts have been extended to March 2017.
Focus has been given to dealing with rough sleepers in the City during the last 6 months, with direct referrals into some schemes from RS services, and remodelling of Brydon Court. Other options will be considered with RPs.
There has been continued focus with Move On including how the Brokerage service can better support Providers in services e.g. training, consultancy sessions or surgeries at accommodation schemes. Also plans to respond to some of the concerns rose via Manchester Move and Northwards about the readiness for independent living of people leaving temporary accommodation and housing related support, and the availability of resettlement support. Actions: (a) Jane to feedback on plans for a T&F group to pick this up. (b) Jane to feedback on audit findings in socially excluded schemes and in-house homelessness provision. These findings will provide a better understanding of the numbers of people affected by RRL and SOP including those who are ready to move on.
Work is progressing with M-Think - shared database which is seen as a very useful tool. Plans already in place for rough sleeping services and to support the YP pathway, and now exploring whether it can be used in the general sector too.
Jane passed around a copy of Sleeping Rough - The Ugly Truth booklet which was produced in partnership between Sanctuary and Eastlands. The content of the booklet has been written by residents at Victoria House. Additional copies are available from Eastlands.
4. / Update on developments across health and social care
(a) The One Team
James Williams delivered the presentation which was well received. Slides will be circulated following the meeting.
(b) The Care Act 2014 and new duties around suitability of housing
Zoe Robertson delivered the presentation which was well received. Slides will be circulated following the meeting.
5. / Safeguarding
Cath delivered a presentation which was well received. Slides will be circulated following the meeting. The new early help hub service will be launched on the 28th Sept.
6. / Alliance Update
(a) Young People
Lisa advised the last meeting on 4 September received a good presentation on Inspiring Change programme which linked this into the Brokerage service. M-Think project for young people and the issue of emergency beds are currently being discussed. Work ongoing on development of referral pathway through YP accommodation and support services.
(b) Mental Health
Collette advised that she had not been to an Alliance meeting. Action: Clare Shaw from Creative Support to provide an update for the next meeting.
(c) General
Nothing discussed.
7. / Homelessness Update
(a) Rough Sleeper Plan
Jane briefly outlined the current overview of rough sleepers in the City. Actions: (a) Jane to circulate the plan to members. (b) Members to send Jane any examples of how HB is being used by Providers in other LA’s to fund community based support services.
