Instructor Employment Application &Course and Instructor Approval Biography Form

This call for instructors and course proposals will remain open indefinitely. We welcome inquiries from prospective instructors in all fields.

Course and Instructor Approvals:

For all course and instructor approvals, UC Berkeley Extension is required to submit the following documentation:

  • Instructor Employment Application & Biography Form: Instructor applicants are required to submit Instructor Employment Application & Biography Forms in order to process approvals to teach courses which they have not taught before at UC Berkeley Extension. Instructor applicants must complete and return the forms via fax or mailto their Extension representative within two weeks of Extension's request. Forms are also accepted via e-mail if the attachmentsinclude original signatures that are scanned into PDF files. (This is not necessary for instructor applicants teaching the same courses again. A new Instructor Employment Application & Biography Form will be required at least every three years.) Note that references cannot include the course program director.
  • Three Letters of Reference must be included in the approval packet. Extension representatives will send letters requesting a reference to each of the three names that instructor applicants provide on the Instructor Employment Application & Biography Form. If there could be delays in securing a reply from one or more of the references listed on the Instructor Employment Application & Biography Form, please suggest some additional references to assure that the Extension representative will have three letters to submit by the deadline. References should be able to address the instructor applicant's familiarity and related professional and academic experience with the proposed subject matter, as well as their prior instructional experience. Instructor applicants should consider the audience, who will be reading these references, when they provide the contacts.
  • A Course Outline is required for all new courses and continuing courses to be taught by a different instructor. Instructor applicants should consult with their Extension representative about the deadline. The course outline is the basis for the instructor’s course syllabus.

Terms of Employment:

The process of course and instructor selection, academic approval, scheduling, and hiring can take up to four months. Appointments are by agreement for the length of the course. The University makes no commitment to hire an instructor until it has sent and received a signed instructor agreement. The instructor agreement is contingent upon the University's formal academic approval of the course and instructor. Should an instructor's course section cancel for any reason, the instructor agreement also cancels—including rights to compensation. The instructor agreement is between the instructor and the University, and instructors may not substitute another instructor to meet any portion of their teaching commitment to the University. All events and preparation leading up to the teaching assignment are assumed as pre-course responsibilities of the prospective or returning instructor, without remuneration.UC Berkeley Extension does not sponsor work Visas for instructor positions and all applicants must be able to prove eligibility to work in the United States at time of hire.

An Extension instructor applicant only becomes an Extension instructor for a given course in a given term when he or she completes the following three processes. It is imperative that these processes are completed each term prior to the course start date:

  • Academic Approval Process: The course and the instructor must be academically approved by the University. The following Instructor Employment Application and Biography Form is used in the academic approval process.
  • Employment or Independent Contractor Status: University Extension will ask you to provide documentation to verify proof of your identity and eligibility to work in the United States.
  • Employee Instructor: The instructor applicant must complete an Employment Packet to become an employee instructor. This requires filling out employment forms, and being present to sign in our offices in Berkeley or San Francisco. Arrangements can also be made to mail in the employment packet, but only if the paperwork is officially notarized.

  • Independent Contractor Instructor: The instructor applicant can review and complete the required documentation to be considered as an independent contractor.
  • Instructor Agreement: Instructors must have an original signed instructor agreement for each course section on file with UC Berkeley Extension prior to the class start date.
