Instructions to Applicants for

Specialist Anaesthesia Training Posts Year 3 – 6 (SAT Year 3 – 6)

Please read this information carefully before completing your application

GENERAL POINTS
  • Only the information contained on the application form is copied for consideration by the short-listing and selection panels. Therefore, donot send a copy of your Curriculum Vitae with your application.
  • Applications are accepted only if received inelectronic format. Sealed references and other documentation must be posted to the College to arrive prior to the closing date. Incomplete applications will not be accepted. You will be informed if your application is incomplete if it is receivedfive working days before the closing date.
  • All applications must be accompanied by a €100 application fee
  • An application will be considered incomplete and will be returned if any of the required documents are missing:

Required Documents:

  1. A copy of your current Medical Council of Ireland Registration Certificate (do not send the original)

and

  1. Documentary evidence of having passed the Primary FCAIand Copies of all CAI in-training assessments during BST for each period of BST must be provided. CAI is not responsible for supplying these.

Or

  1. If your qualification is notThe Primary FCAI, a certified copy of an acceptable equivalent qualification is required, as published in the College’s Examinations Regulations

and

  1. If you do not hold a Certificate of Completion of Basic Specialist Training (CCBST)issued by the College of Anaesthetistsof Ireland, a certified copy of evidence of completion of acceptable equivalent basic training is required. In addition certified copies of the equivalent of an in-training assessment for each period of BST must be provided.
  2. Onepassport-size photograph with your name in block capitals on the back must be included.
  3. You must send two references in signed, sealed envelopesto be received prior to the closing date for applications.
  4. Any other documentation relevant to your application.

NOTE:

Original documents should not be submitted, only copies. *All copies of documents submitted should be properly certified in the manner described in Addendum 1. In additionthe College of Anaesthetists of Ireland reserves the right to request certified copies of Primary FCA Certificate and/or CCBST Certificate issued by the College of Anaesthetists of Ireland where it deems necessary.

Canvassing in order to obtain an unfair advantage at interview is unacceptable.

Incomplete and/or late applications will be returned to the secure address, as entered on the form submitted.

SECTION A – Personal details - Enter these as on Medical Council/College documents

  • The details given in this section must correspond with your personal details as they appear on the Medical Council and other College documents accompanying your application.
  • If you have been previously registered with the CAI or have made any attempts at sitting the CAIExaminations you must enter your College IdentificationNumber in this section of the application Form.
  • Please ensure that you give a current secure address and details of how you may be contacted quickly.

SECTION B – Medical Council (Ireland)Registration

  • You are asked to provide your medical council registration number.
  • All applicants must be registered with the Medical Council of Irelandor provide evidence from the Medical Council that their application for registration is under way. The letter must state that the Medical Council has decided that your application and documents permit you to apply for trainee specialist registration
  • The HSE now requires that all doctors being recruited to positions in the Irish health serviceare required to be proficient in the English language.All applicants applying for positions through the training bodies must provide the following:

(a) confirmation of completion of a medical degree in a country where English is the primary language spoken by the vast majority of the people of that country.

or

(b) IELTS (International English Language Testing System Academic Test) Certificate with an overall band score of 7.5 and a minimum score of 7.0 in each of the four domains on the academic test.

SECTION C – Examinations

Undergraduate Examinations

Clinical Experience

BST Record

  • A certified copy of a qualification equivalent to Primary FCAI, must be included with your application.
  • Details on your training programme must be supplied here. If you are not on a programme please tick other.
  • Applicants who hold a Certificate of Completion of Basic Specialist Trainingissued by the College of Anaesthetists of Ireland do not need to submit a certified copy of this qualification. If you have not yet completed BST training you must provide evidence of being eligible for CCBST at the start date of the post.
  • For applicants who underwent basic trainingin the UK, a certified copy of a letter from the Royal College of Anaesthetists or equivalent College stating that you have been awarded the CCSHOT or equivalent in anaesthesia is required. Certified copies of the equivalent of In-training assessments for each period of BST must be provided to gain marks in this section.
  • Copies of CAI In-training assessments for each period of BST must be provided. It is not the responsibility of the CAI training office to supply these.

Employment History

Beginning with the most recent(i.e. your current position) list, the location of all previous appointments. In each case, indicate the grade, the specialty, the dates and duration in months

Highlight clinical experience relevant to this specialty including clinical practice, teaching experience, audit and management experience. It is recommended that you use the headings listed above to categorize the information you are providing. You may add and/or substitute others if you wish.

Non Mandatory Clinical Skills Courses

  • Fill in non mandatory clinical skills course here e.g. ATLS, ATLS, PALS etcMandatory simulation courses or other mandatory courses are not relevant here.

  • If you have presentation, publications in peer review journals (Abstracts and Full text) and published audit please list these with full bibliographical details.

SECTION F – Aims and Career Objectives

  • In this section, you are being asked to explain how you have sought to prepare yourself for this post, and how the particular programme for which you have applied fits into your career plans and objectives

SECTION G – Additional Information
  • Highlight any personal achievements which you consider significant or include any additional information. You will have an opportunity to expand in this section at the interview stage.

SECTION H – References
  • Please include two references with your application, one of which must be from your most recent supervisor. You are required to ask the referees to complete the reference form and return it to you in an envelope signed by the referee across the seal. You should ensure that these two envelopes reach CAI by the closing date for the relevant post.

SECTION I – Notes
  • Read each note carefully and confirm your understanding of each and everyone.
  • Please read the three declarations carefully and sign and date your agreement with the text of each declaration. An electronic signature is appropriate here.

SECTION K – Application Checklist and Signature
  • Confirm here the documents that you are including with your application, (and check that they have been attached). Be advised that applications which are incomplete (see General Points) will be returned and, unless re-submitted correctly and in time, will not be put forward for consideration by the shortlisting and appointment panels.
  • The application form must be signed by you and dated (dd/mm/yyyy format). An electronic signature is appropriate here.

RECOMMENDATIONS
  • We recommend that you submit your application as early as possible. In the event of your application being judged incomplete you will be notified and you will then have time to address the problems and to re-submit the properly completed form before the closing date if application is received five days before the closing date. Youmust submit your application form by email apart from the references and other documentation, which we advise should be submitted by recorded delivery.
  • If you are delivering reference envelopes by hand, you should have these date-stamped by a member of staff at the College.
  • If you are still uncertain about how to complete any part of the application form, or if you are unsure about anything covered in the foregoing instructions, please contact the Training Officefor advice.
  • Please note the College of Anaesthetists of Ireland reserves the right to retain a reserve list following interview.

Addendum 1

“Certified” Copies

If you have been asked to provide certified copies of documents submitted as part of your application, please proceed as follows:

To have a copy certified, you must present the original document and the photocopy to one of the following:

  • A Fellow of the College of Anaesthetists of Ireland of the Royal College of Anaesthetists.
  • A member of the Garda Síochána at a police station
  • A Commissioner for Oaths
  • A Notary Public

The copy must be certified with the following words or an appropriate equivalent (BLOCK CAPITALS, please)

I (NAME) certify that this is a true and faithful copy of an original document which I have seen.

It must then be signed, officially stamped and dated. In the case of a document being certified by a Fellow of the College, the stamp of the hospital must accompany the signature.

Do not send the original documents. College of Anaesthetistsof Ireland will not take responsibility for their loss or damage.

Those who applied in previous years should note that it remains theapplicant’s responsibility to resubmit certified copies each time they apply. The Education and Training office(which processes all the large number of applications received in the College) has not the resources to retain or locate such documentation on your behalf.
