Instructions for “Grange Membership Recognition Application” Form
Secretaries, please allow four weeks for processing the order. It should take less time than that.
Use the gold “National Grange Membership Recognition Application Form”, dated June, 2009. Make sure it is completely filled out, including the “SHIP TO” name and address. This is where seals, certificates and/or letters,and otherordered items will be sent. Affix your Grange seal in the space provided. This form and these instructions were sent to Subordinate/Local Secretaries in June 2009. They are also available on line at under 'Forms”, then 'Membership Recognition'. If needed, request more forms from the Ohio State Grange, or you may copy them. Make this check payable to “NATIONAL GRANGE”.
IF ordering Silver Star Certificates and/or 40, 55, 60, 65, & 70 year seals, use the NEW OSG Jewelry Form dated June, 2009. Make that check payable to “OHIO STATE GRANGE”.
Send:(1) the completed “National Grange Membership Recognition Application” form, dated June, 2009,
(2) a check (made out to “National Grange”) for the amount figured on that form,
And – IF ordering Silver Star Certificates and/or 40, 55, 60, 65, & 70 year seals or other items from OSG,
(3) the completed OSG Jewelry Form, dated June, 2009,
(4) a check (made out to “Ohio State Grange”) for the amount figured on that form.

- The Ohio State Grange will send the requested item(s) to the name and address listed on the form.
- The OSG Secretary will record the information, forward the form and check to the National Grange, and sendthe State Master’s letters, seals and Silver Star Certificates to the name and address listed on the form.
- The National Grange will record the information and send the requested item(s) to the name and address listed on the form.

Appropriate Golden Sheaf pins and membership recognition pins are available from the Ohio State Grange or from the National Grange. To order from the Ohio State Grange, use the OSG Jewelry Form dated June, 2009. This form is on line at “Forms' then “Store” and was also sent to Secretaries in June, 2009. Bear in mind that items ordered from the Ohio State Grange will be charged 6.5% Ohio sales tax; items ordered from the National Grange will not be charged any tax. A shipping/handling charge will be applied to items ordered from both places (except seals and certificates ordered from OSG). The National Grange order form is in the printed Grange Store book, or you may order on line at under “Grange Store”.
Shipping, Handling, and Tax Information
Items ordered from the National Grange are sales tax free plus shipping and handling.
All Silver Star Certificates and 40, 55, 60, 65, & 70 year seals MUST be ordered directly from the Ohio State Grange and will be charged 6.5% tax and mailed to the name and address listed on the form with no shipping and handling charge. Checks for Silver Star Certificates and seals must be made out to “OHIO STATE GRANGE”.
Other items ordered through Ohio State Grange will be charged 6.5 % tax plus shipping and handling.
All ordered items picked up at the State Grange Office or at a Grange event will be charged 6.5% tax with no shipping and handling charge. In this case, please let the Ohio State Grange know to bring your items to that event.
All items purchased at the State Office or at a Grange event from our inventory will be charged 6.5% tax with no shipping and handling charge.
The Ohio State Grange shipping/handling charge is: Orders: Up to $10.00 = $4.00; $10.01-$19.99 = $5.00;
$20.00- $49.99 = $7.00; $50.00-$74.99 = $9.00;$75.00-$99.99 = 10.00; $100.00-124.99 = $12.00
The National Grange shipping/handling charge is: Orders: Up to $10.00 = $4.00$; 10.01-$19.99 = $5.00;
$20.00- $49.99 = $7.00; $50.00-$74.99 = $9.00; $75.00-$99.99 = 10.00; $100.00-124.99 = $12.00;
$125.00-$249.99 = $15.00; $250.00 and above = $20.00
