Instructions for filling out NHS forms

1. Remember that these forms will be used by the selection committee to further review your

qualifications for induction into the NHS. It is important to remember that having good grades does not automatically qualify you for induction into the society.

The society is committed to four ideals; that of scholarship, leadership, service, and character. These forms are to help the selection committee weigh your strengths in each of these areas. All of these areas carry equal weight, so it is important that you choose the proper people for recommendations.

2.  Select four current staff teachers to fill out the NHS evaluation form. Please put your name, the teacher’s name and the class that they are to evaluate. Give the forms back to Ms. Ludford, not the teachers.

3.  Complete the application form. It must be typed. You must have at least two items in each category.

4.  Type your essay: Discuss/Describe a service project (short- or long-term) that you’ve participated in or a leadership role that you have held and its impact on you or your community/school.

5.  You must have one letter of recommendation from outside of school (clergy, employer, etc.). No relatives. Have the letters sent to: Christine Ludford, Central High School, Box 247, Norwood, MN 55368. Ms. Ludford must receive these by February 19. You may wish to give them an envelope already addressed and stamped. You could have it e-mailed to me: .

6.  These forms are due back to Ms. Ludford by 3:05 February 25.

The process of induction is as follows:

a.  Students must have a cumulative GPA of 3.5

b.  Students must submit four teacher recommendations

c.  Students must complete application and paragraph

d.  At least one letter of recommendation from the community must be completed

e.  Students must get a minimum of three votes by the faculty review committee.(There are five members on this council)

Once selected, students do not automatically remain members of the NHS. All students are expected to maintain the same standards that allowed them to be selected. Any student not maintaining these standards will receive a period of time to correct their shortcomings, if not corrected in this time span, the student will be removed from the NHS. Once removed from the society, a student is never again eligible for membership.
