Answer questions H1 through H11 for each site activated for SSO:

H1. Select the site eligibility type established for SSO, following the SFSP regulations.

  • Open Using School Data: select for open sites that meet the 50% F/R eligibility or are in the attendance zone of a school that meets the 50% criteria
  • Enter the SNP# of this site and the October F/R eligibility % from the DOE report
  • Or, if this site is located in the attendance zone of another school, then enter that school’s SNP# and October F/R eligibility % that allows this school to qualify
  • Restricted Open: select only for a site that meets the “Open” criteria but will restrict access on the basis of security, safety, or control. Detail the reasons for the restricted site in the text box below.
  • Open site using migrant organization: Certification from the migrant organization must be submitted to DOE-SNP to document that the site serves children of migrant worker families and that it primarily serves migrant children if it also serves non-migrant children.
  • Closed Enrolled Site: Documentation the same as an open site (including SNPU# and F/R%) and, if the individual enrolled children’s eligibility from DC and applications is being used, detail the source of the information (from their regular school year school, applications submitted at the site and approved by the LEA or a combination). An explanation of why the SFA is sponsoring a closed enrolled site must be provided in the text box. The free enrollment, reduced enrollment, and total enrollment of the Closed Enrolled site must be provided.

H2. Select the months of operation for the SSO (May and June only are activated in the current 2016-17 Agreement. July and August will be activated in the new year agreement for 2017-18).

H3. Select meal type(s) that will be served. There is a limit of 2 unless the site is a camp or migrant site.

H4. Estimate the number of children that will be served daily.

H5 through H8. Select the counting procedure that will be used, for each meal type offered, from the dropdown box.

H9. Indicate if another organization operates the site (Parks and Recreation, Library, etc…)

H10. Describe how the SFA will notify the community of the availability of meals to children 18 and under, including the hours, days, weeks of operation and the location.

H11. Indicate if this site will claim meals for another site.
