Instructions / Quilt Turn in: 4/20/13 - Location to be determined and announced in March 2013.
1.  Use one form for each quilt registered.
2.  Quilts must have a label with at minimum, name of quilter. It must be clean, smoke and pet hair free.
3.  Each quilt is to be bagged and labeled in a Ziploc Big Bag size appropriate for your quilt. We will strive to ensure that you receive your original Ziploc bag returned with your quilt.
4.  Before placing your quilt in the bag, please fold it so that the quilt label can be seen through the bag.
5.  A 4” sleeve must be securely attached to the back of the quilt. (Note special sleeve instructions for Guild Challenge quilts.)
6.  Quilt size must be accurate – within 2” as quoted on this form. If not, there is a chance that your quilt will NOT be displayed under your chosen category and will be hung in an area that has extra space. Please measure carefully.
7.  Attach a color photo (preferably no larger than 4x6, UNLESS your quilt is for sale; then please submit an 8x10 photo for placement into a notebook to be located at the auction table) to the back of the registration form. Print your name, phone number and the name of the quilt on the back of the photo. Photos will not be returned.
8.  If you are entering more than one quilt, we will do our best to include them all. However, if there are space issues, we may have to reduce the number of quilts displayed. We will contact you if this is necessary in order for you to prioritize.
9.  Completed registration forms and pictures can be submitted three ways:
a. Emailed (preferred method) to: - please make sure you attach your digital photo; OR

c. Dropped off at the March guild meeting.

Show Challenges Entry Information – SELECT ONLY ONE PER FORM
Guild Challenge – “Tutti-Fruiti” – Size: Must be square or rectangular, 35 - 45" on a side (maximum 160" perimeter),
to be included in the AQS Ultimate Guild Challenge.
Vendor/Bloomin’ Challenge VII – “Geometry” – Size: Quilts and wall-hangings may be any shape with a maximum perimeter of 160”. Wearables may be any size. Other articles, such as purses or stuffed animals are also suitable for this challenge.
Service Project Challenge – “Quilts from the Heart – Size: At least 36" x 36" for a young child, or at least 42" x 52" lap size for an adult. Maximum size for a lap quilt is 72" x 72". Quilt must be quilted by hand or machine, not tied. These quilts will be donated to the Fisher House of Walter Reed National Military Medical Center at Bethesda.
