Instruction for Docket Summary Cover Sheets.

GENERAL INFORMATION FOR ALL COVER SHEETS:
Style of Docket: Put the style that you want here. If it needs to be changed, that will be done in the Secretary’s Office.
Petitioner/initiating party: Name(s) of company(ies) filing the application. If the company is not licensed before the Commission, we need contact information for the company, address, phone, fax and e-mail address.
Attorney(s’) name, address, phone, fax and e-mail: self explanatory (SEE NOTE BELOW REGARDING COMPLAINT DOCKETS)
Rule 3.04 entries for service list: You may use the attorney(ies) above as your service list entries, but you may also use a company representative and one attorney or any combination that you choose. There will ONLY be two entries on the service list for each company though.
Write a brief statement…. This needs to be more than “rate case”, “application for certificate of public convenience and necessity”, etc. In this area, we don’t want you to repeat your entire application; but we do want a general description of the action that you are filing at the Commission.
FOR “ALL OTHER” COVER SHEETS
Nature of Action: Choose at least one but not more than three categories for your new docket.
Docket designator: U is the most common docket type at the Commission. Most “A” and “R” dockets are commenced by the Commission. “P” dockets are used for promotional dockets.
FOR “TF” COVER SHEETS: Be sure that you answer questions 1-5 of this sheet completely. For instance, question 3 asks for both a dollar amount and a percentage.
FOR “C” COVER SHEETS: We recognize that many individuals who file complaints are not represented by attorneys. We also recognize that some of this language may be confusing to them. If you need assistance completing the form, contact the Secretary of the Commission at 501-682-5782.
FOR “RATE CASE” COVER SHEETS: Questions 1 – 6 will expand to give you additional room for answering the questions. These questions must be fully answered on the cover sheet. Question 7 allows for ample room for you to “identify the major retail revenue requirement increase drivers” for the Commission. Please be sure to use that expanded area to complete the information.
