<Insert Group Name> is pleased to assist you in your efforts to improve your health and well-being. The Walk for Health Challenge is an exciting voluntary walking incentive program available to any <Insert Group Name> Employee.

Participants of the program receive incentives and can earn prizes whenever they walk at home or at work. The program encourages participants to get 30 minutes of physical activity on most days of the week. The top two participants with the highest level of walking participation are rewarded with a prize incentive. At the end of the 90 day program (June 1-August 31) <Insert Group Name> will collect walking journals from all participants to determined who will be awarded the grand prize. ($Insert value)

How do I join?

On <Insert Date>, <insert registration process> eg. An HR representatives will be available to register interested participants.

Place / Time

How does it work?
It’s easy. First, the program has been divided into three sessions (see details below). These sessions are four weeks long and run consecutively after one another for the total 12-weeks.

After you sign up, begin by recording your fitness walking minutes on the Walk for Health log sheet for Session 1. At the close of the session, submit your log to Stephanie Bouchard.

Please Note: Minutes leisurely walked throughout the course of the day at work or at home should not be recorded or counted on your walking log sheet. Such walking is not considered fitness walking for the purpose of this program.

All <Insert Group Name> Walk for Health Challenge participants who complete the entire 12-week program will be eligible for the grand prize drawing. The top two walkers will be selected to win the grand prize valued at $<insert value>.

Goal: Walk a minimum of 800 minutes.

Goal: Walk a minimum of 1,000 minutes.

Goal: Walk a minimum of 1,200 minutes.

In order to be eligible for the Walk for Health Challenge incentive, you must:

• Submit your Walking Log after the completion of each Session to your Human Resource Specialist <Insert Name>.

• Submit your Walking “certification” form along with each session walking log to your Human Resource Specialist-<Insert Name>.
