Information is being collected for the evaluation of the National Disability Insurance Scheme (NDIS) trial, which is being conducted by a consortium of evaluation, survey research and disability experts led by the National Institute of Labour Studies (NILS) at Flinders University of South Australia, contracted by the Australian Government Department of Social Services.

The evaluation of the NDIS trial will collect important information (including personal and sensitive information) to help the Australian Government understand the impacts of the National Disability Insurance Scheme on people with disability, and their families and carers; the disability sector and its workforce; mainstream services and providers; and the wider community. This will help strengthen the NDIS, as well as other disability services and supports. Information will be collected using a variety of approaches including paper and web surveys, telephone interviews and face-to-face interviews.

NILS and its consortium partners are collecting information from people with a disability, and their nominees (if applicable), carers, and people working in the disability sector. This information is collected on a voluntary basis and will only be used for:

Evaluation purposes;

Statistical purposes; and

Maintaining contact with evaluation participants for the purposes of the evaluation.

In compliance with the Privacy Act 1988 (Cth) all information collected will be kept strictly confidential. Data released to the Australian Government Department of Social Services will not allow individuals to be identified.

Provision of information is voluntary. There are no consequences of choosing not to participate in the evaluation. You may also choose to take part in some but not all aspects of the evaluation.Should you withdraw your consent, NILS will no longer collect personal information from you from the date you advise that you withdraw your consent. However, data collected from you before this date may continue to be used and form part of the evaluation of the NDIS trial, however only in accordance with the Privacy Act as outlined above.
