PLEASE READ THE FOLLOWING AND SIGN IN THE SPACE PROVIDED, IF YOU AGREE: As a part of the consideration for the renting of the equipment as listed herein, I the undersigned, for myself, my heirs, representatives, and assigns, recognizing that skating is a potentially dangerous activity, hereby fully and forever release and discharge INDYSPEED, and all its officers, directors, employees, and agents and any and all persons whomsoever directly or indirectly liable, from any and all claims, demands, actions, caused of action, at law or in equity, including negligence, arising or resulting from personal injury or property damage suffered or sustained by me as a result of my use of equipment rented from INDYSPEED.

I do hereby expressly indemnify and hold harmless IndySpeed, and all of its officers, directors, employees and agents, from and against, any and all loss, liability or expense, including reasonable attorney's fees, arising or resulting from the activities described above. I alone assume liability for damage to any persons or property resulting from the use of this equipment. I agree to reimburse IndySpeed for damage or loss from any cause whatsoever of any equipment listed herein. This equipment is at all times the property of IndySpeed. IndySpeed assumes no risk of injury to the user, or any liability for misuse or in the course of use. The terms of this release are contractual, are not a mere recital, and contain the entire agreement between the parties hereto.

I promise to return the equipment on or before the said day, and pay all rent due for any rental exceeding the prescribed date on the present contract Rentals not returned on that day, will accrue rental fees for month not returned until Full Retail price is equaled. Skates vary in price from $300 -$900. Renter will assume all costs of recovery, which are incurred by IndySpeed.

I accept to use the said equipment as is, and while in my possession, I promise to take care of the equipment at all times, and to compensate INDYSPEED for all damages and loss. I declare that the equipment assigned has been adjusted to conform with the norms established for my weight, shoe size, and my experience according to the information stipulated by myself on this contract. Only the person renting this equipment is to use it.

Guardians: I verify that I am the Parent or Guardian of the minor. I have authority to enter into this agreement on behalf of the minor. I agree to be bound by its terms. I accept full responsibility for all medical expenses incurred as a result of the minor's use of this equipment and the skating rink's facilities, and I agree to indemnify and hold harmless the RELEASES from any claim brought by, or on behalf of, the minor.

THE INDIVIDUAL MUST SIGN THAT THEY HAVE READ, UNDERSTOOD, AND AGREE TO THE DISCLAIMER.
