Rate change effective 3/1/09

Periodically, it is necessary to adjust our health coverage rates. As of 3/1/09, the monthly rate on your <insert name of plan> plan will change from <insert current rate> to <insert 3/1/09 rate.

What do you have to do?

To continue with your current medical plan, you don’t need to do anything -- just keep paying your premiums on time.

Our team is here for you Monday – Friday from 8:30 a.m. - 7:00 p.m. and we will be happy to answer any questions you may have.For future reference please take note of our new member service hours.

What are your other options?

If you have been covered for at least 18 months under an Anthem Blue Cross Life and Health Insurance Companyindividual health care plan, you may be able to transfer, without medical underwriting, to another plan providing equal or lesser benefits. Keep in mind that if you decide to switch to another plan, you may not qualify to change back to your original plan.

Did you know that we have other SmartSense plans available that offer a lower plan premium than the comprehensive prescription plan you are currently on? The SmartSense 500 with generic prescription drug coverage and the SmartSense 1500 with comprehensive prescription drug coverage both offer a lower plan premium. You may be eligible to make a plan change now, even if you haven’t been on your current SmartSense plan for 18 months. SmartSense plans offersome of our most affordable premiums.
