1. Does the insured lease employees to other employees?
2. Does the insured equipment to others?
3. Does insured have past, present, or discontinued operations that involve(d) storing, treating, discharging, applying, disposing, or transporting of hazardous material (e.g. Landfills, wasted, fuel tanks, etc.?)
4. Are athletic teams or events sponsored?
5. Does the insured own or operate any other businesses?
6. Does insured own any other property under the same name that is NOT insured?
7. Does applicant own, operate or lease aircraft, watercraft, docks or floats?
8. What is the radius of Insured’s operations?
9. Does Insured perform any snowplowing on public highways?
10. Does insured do any off-season work other than his usual operations?
11. What are receipts for subcontracted work?
12. Work done in/around nuclear plants, power plants or petrochemical plants?
13. Does insured do work in/around hazardous materials abatement such as asbestos, lead-based paid or PCB’s?
14. Does insured use ant type of crane, stand alone, truck mounted crane or bucket truck?
15. What is the insured’s percent of work around hospitals?
16. Does the applicant draw plans, designs, or specifications for others?
17. Any work performed underground, above 15 feet, or do they utilize scaffolding?
