Transition Advocate

Moving People from Nursing Facilities to the Community

Site Location: Based in Santa Barbara

Salary/Benefits: Part-Time: 28 hours/week $18/hr. 9 month grant funded position

Essential Functions Of The Job:

·  Make contacts/develop working-relationships with nursing facility social service workers throughout Santa Barbara County and periodically visit each location; and present the transition program to social services and discharge planning staff

·  Respond to and screen transition candidates responding referrals, and present the transition program to residents;

·  Provide initial transition information, options counseling, orientation and assessments and transitional care plans required by the program;

·  Assist participants setting and attaining goals identified within their care plans;

·  Work with other ILRC staff to assist transition participants with IL services;

·  Perform all related record keeping, consumer charts and data collection activities;

·  Monitor transitions, maintain contact, provide peer support to participants and handle reporting to Transition Coordinator for those consumers on the CCT MediCal program;

·  Work with Transition Coordinator and Program Manager to ensure accurate reporting and billing;

·  Provide program narrative and consumer stories when required;

·  Promote harmonious working relationships and participate fully as member of working team and conduct agency business in professional manner;

·  Other duties as assigned.

Essential Qualifications:

·  Have and use excellent communication and organizational skills;

·  Reliable transportation and ability to travel throughout the county;

·  2 or more years experience in related field;

·  Experience working with persons with disabilities;

·  Familiarity with Skilled Nursing Facilities

·  Ability to meet deadlines, balance multiple tasks;

·  Ethical work practices: honest, dependable, team-oriented work style.

·  Ability to arrange for/provide own transportation throughout service area.

Preferred but not required:

Personal experience with disability

Bilingual English/Spanish

Nursing background helpful
