MAILING OF THIS NOTICE
As Residential Parent, I
_____have no objection to the Court mailing a copy of this notice to the Non-Residential Parent at his/her captioned address.
_____state that the Non-Residential Parent has been convicted of domestic violence or child abuse therefore objects to the Court mailing a copy of this Notice to the Non-Residential Parent AND REQUESTS THAT THIS MATTER BE SET FOR HEARING on the issue of whether a copy of said Relocation Notice should be mailed to the Non-Residential Parent as set forth in ORC 3109.051(G)(2).
IMPACT ON VISITATION SCHEDULE
As Residential Parent, I
_____acknowledge that no change in the current Parenting Time Schedule is required because the Non-Residential and I are currently subject to the Court's Parenting Time Schedule and the same will still continue be applicable.
_____agree that upon the relocation taking effect, that the non-Residential Parent's visitation shall be governed by this Court's Long Distance Parenting Time Scbedule, a copy of which is attached hereto as Exhibit A.
_____do not agree to the Long Distance Parenting Time Schedule being applied and REQUEST THAT A HEARING BE SCHEDULED on the issue of whether it is in the best interest of the child to revise the current Parenting Time Schedule.

You have the right to request a hearing for the Court to determine whether it is in the best interest of the child(ren) to revise the Parenting Time Schedule in this case.
If you do not request a hearing, the current Parenting Time Schedule may be modified as requested by the Residential Parent as set forth above.
