Staffing Update

We welcome two new members of staff this month.

Kelly Day has commenced working with both the Wellington and East Gippsland PCPs with a brief that includes the GP Engagement Strategy from the Improving the Client Journey Strategy.

Tara Smark has been with the WPCP for two days. Tara has applied social model of health principles in mental health settings for the past four years. Most recently she has worked for the Mental Illness Fellowship Victoria on a health promotion project that aimed to improve the physical health of people living with serious mental illness in the Mornington Peninsula area. The project developed strategies to improve social support and reduce barriers due to socioeconomic disadvantage in order to reduce the helplessness consumers feel when faced with physical health challenges. Tara developed a number of partnerships with other mental health service providers along with other community services and private organisations in order to improve the resources available to the organisation and its clients. The project resources were widely distributed so that the information would benefit a wide range of stakeholders and continue to build partnerships.

Emily Durbridge finished with the WPCP two weeks ago. We thank Emily for her excellent work in the area of Chronic Disease Management and wish her well in her new role with the CFA in Sale.

General Business

The 2010 /2011 PCP reporting process is currently in full swing and will be completed by 30 September 2011. The process is largely based on case studies and is similar to last year though we understand there will be some tweaking to the template which has not been released yet.

I would like to thank agency staff in advance for their support in this process. I know it can be time consuming but it can also be a way of reflecting on our work and looking at ways to improve it.

The process to update the SCTT Tools has been in progress for some months. In July at the two SCTT 2012 Revision project Steering Committee meetings recommendations were put forward and are awaiting endorsement. Once this happens the new SCTT template formats, based on the recommendations,will be piloted

Chronic Disease Management Network

Apologies for any inconvenience caused by the cancellation of the CDM Network Meeting on2 August. I will be approaching members of the network individually to determine what they would like to get out of a CDM network and what this might look like. I am thinking that a Community of Practice style workshop four times a year with a dedicated professional development activity maybe of more use to the PCP Community. I will be in touch in the next few weeks.

Medicare Locals Update

The ITA for the Gippsland Medicare Local was completed and submitted in July as required. If successful, Gippsland will be looking at the transition to the new structure after October. At the WPCP Business meeting I was asked what the model looked like. I have therefore attached a short presentationdelivered to South Coast PCP by Rhett McLennan, CEO of South Coast GPA. Rhett has kindly agreed to share it with us and it provides a clear picture of the proposed Gippsland Model and some information about the process and the future.

Arthritis Victoria Community Speakers presentation for your community groups

Arthritis Victoria has a number of Community Speakers across Victoria who present to community groups, organisations and rotary groups etc. The Community Speaker’s presentation includes an overview of the common types arthritis and musculoskeletal conditions as well as information on prevention and management of the condition.

Arthritis affects a large population of people of all ages and therefore I believe there will be a few people in the Strength Training Group who live with some form of arthritis or a musculoskeletal condition. Arthritis Victoria receives positive feedback about the Community Speaker Program and hopefully some of the Strength Training Groups will see the benefit in learning more about arthritis and musculoskeletal conditions.

Working Together Conference - implementing chronic condition self management in practice

The conference program will feature key note presentations by Professors Kate Lorig (USA), Malcolm Battersby (Australia), Dr Patrick McGowan (Canada) and Susan Reid (New Zealand).

The conference will offer service providers, researchers and consumer representatives the opportunity to hear about advances in the different approaches to chronic condition management (including the Stanford and Flinders Programs), health literacy, evidence based practice in action and men's health.

Community and Primary Health Care Nursing Conference 2011 - Leading the way to local care

Climate Change and Public Health

This seminar, organised by Centre of Research Excellence in Patient Safety (CREPS) Monash University, will address the potential impact of climate change on public health.

The question 'Why is climate change a public health issue?' will be addressed, and speakers will discuss potential direct and indirect health impacts of climate change, including extreme weather events, vector-borne infectious disease, food and water quality, air quality and mental health. Adaptation and mitigation initiatives will be discussed, including the co-benefits of these for health.

New Pilot: Rheumatology Help Line-launch - 6 June 2011

Arthritis Victoria has launched a pilot Rheumatology Help Line. The help line, made possible with funding from the Victorian Department of Health, aims to increase access to accurate evidence-based health information to people with a musculoskeletal condition, their caregivers and other health professionals in the community.

A registered nurse is available on the help line to provide:

  • medical information regarding musculoskeletal conditions and commonly prescribed treatments
  • assistance navigating the complex health, disability and social services systems through the development of common navigation pathways
  • emotional support and advice regarding diagnosis of a musculoskeletal condition
  • linkages to a range of community resources in the caller's area
  • information and support to health professionals providing care to people with musculoskeletal conditions.

The nurse has direct access to rheumatologists at a major Melbourne tertiary hospital for advice and support as required.

The service is available to all Victorians via phone or email 10am-3pm, Monday to Friday during the 9–month pilot period, with a particular focus on providing assistance to those living in rural Victoria and experiencing health inequalities.

Most Significant Change Training in Sale

What is MSC?

MSC is a form of participatory monitoring and evaluation. It is participatory because many project stakeholders are involved both in deciding the sorts of change to be recorded and in analysing the data. It is a form of monitoring because it can occur throughout the program cycle and provides information to help people manage the program. It contributes to evaluation because it provides data on impact and outcomes that can be used to help assess the performance of the program as a whole.

MSC uses an inductive approach, through participants making sense of events after they have happened. With MSC, participants are actively encouraged to exercise their own judgment in identifying stories of change. This involves the use of open-ended questions such as: “From your point of view, what was the most significant change as a result ... Project?” It therefore helps us to capture the expected and unexpected results, the intangible and the indirect consequences of our work while at the same time uncovering the drivers for change in a particular situation.

The training includes:

  • Background and origins of the technique
  • Purpose in monitoring and/or evaluation
  • Practical skills of story collection, selection and feedback.
  • Use of stories
  • Design your own MSC process with input and review by a qualified trainer.

A new body image resource just released;

Elder Abuse Prevention Project Extension

The original funded Elder Abuse Prevention Project is almost complete with only two more community information sessions and three world cafes to be held which are scheduled and work has commenced on the policies, procedures and interagency protocols in responding to elder abuse.

The Project has been extended and we are delighted to welcome Linda McCrorey who will be implementing the extension of the project on a short term contract. Linda has a background in continuous quality improvement activities, consultation and project management. The focus of Linda’s project will be around the policies, procedures and interagency protocols in responding to elder abuse. The role will involve the provision of support to organisations with the development and review of policies and procedures and working with existing networks to explore and develop interagency protocols. The Wellington Shire Council Inclusive Emergency Management Project Steering Committee is looking for people to be involved in the Committee. This is one of three such projects in Victoria.

The desired outcome for Wellington Shire Council of the Inclusive Emergency Management Project will be the development of strategies for managing the needs of people with a disability to prevent or mitigate, prepare for, respond to and recovery from, the impact of an emergency event on the community.

The Committee is seeking people with a disability who live in the community, people who work with people with disability and their families and family members or carers to bring their expertise to this project. The draft Objectives of the Steering Committee are to:

  • Determine the direction of the Wellington Inclusive Emergency Management Project
  • Provide advice and guidance to the Wellington Municipal Emergency Management Planning Committee
  • Development of strategies and resources for the Wellington Municipal Emergency Management Planning Committee
  • Contribute to and participate in the development of educational workshops for community members about emergency management preparedness
  • Contribute to Inclusive Emergency Management Reference Group
