Important Payroll Information for Faculty

We hope you will accept your offer to join us at the University of Colorado. Along with your offer letter, we’ve included related forms.

Submitted with the accepted offer letter:

  1. Personal Data Worksheet (2 pages). Used by the College’s Human Resources Center to enter appointments into the payroll system. Be sure that the name and social security number you enter on the form match your social security card. Your birthdate is a required field. This information is necessary in order for you to acquire such things as a CU e-mail address, access to the library, course rosters, etc., and should be submitted as soon as possible. The Personal Data Worksheet may be faxed to the College’s HR Center at 303-492-4944. Your home address and phone number should be updated once you are settled in the area.
  2. Oath Form. Completion of this form, including having it notarized, is required of employees who are U.S. citizens. Those who are non-citizens must print their names in the box in the upper-right hand corner of the form and submit the form, but it need not be notarized.
  3. Emergency Contact Worksheet. Each employee must designate a primary emergency contact person.

Submitted to your department or other designated party upon your arrival on campus:

  1. Direct Deposit Authorization Form. Direct deposit is mandatory for all CU employees. Your bank may be located anywhere within the contiguous United States. This form and instructions for completion are available on the Payroll and Benefits (PBS) web site at: .
  2. Form W-4. Required for Federal tax withholding purposes. This form and instructions both are available on the PBS web site listed above. If you will not be joining us this calendar year, please wait to complete this form until the year you will begin your appointment.
  3. Employment Eligibility Verification (I-9 Form) All University employees are required to complete the I-9 form by the third day of employment. Standard documentation consists of either a current passport, or a combination of a US driver’s license and an original social security card. Other documentation options are available. Although fall appointments officially begin one week prior to the start of classes in August, faculty do not receive their first paycheck until the end of September. Pay day is the last working day of each month.
