Purpose: The purpose of this survey is for you to assess your organization’s implementation ofquality assurance (QA) and to assist with prioritization of your implementation efforts. The survey includes questions that address organizational development, policy development, and data-based decision making.

Instructions:
Please respond to all questions by selecting a response that best describes how your organization is conducting its work TODAY. Each question has three responses ranging from:

  • Pre-planning/implementation: you have not started doing much, if any, work in this area.
  • Planning/beginning implementation: you have developed or are developing a plan, but are just in the beginning phases of implementation.
  • Advanced implementation: you have integrated this into your daily practice and are assessing the quality of your performance on a regular basis.

Data is used throughout the organization to measure our performance and provide feedback.
