Parent/Guardian Consent Agreement:
I hereby apply for myself or my son/daughter to play cricket with the St John Bosco Cricket Club. I acknowledge in accordance with the St John Bosco Youth Centre’s constitution and the clubs registration and gradings policy, that membership is for myself, or in the case of my child for juniors, this extends to my spouse/partner and I. I also note that the Club will undertake to exercise all reasonable care in the conduct of its activities but it declines to accept responsibility for accidents occurring as a consequence of my child's participation in such activities. By signing this registration form, the undersigned confirms on behalf of the player and the player’s parents or guardians that each of them acknowledges that the SSJCA may publish Codes of Conduct from time to time as applicable to them, including as to the consequences of any breach of same, and that each of the player and the player’s parents or guardians agrees to be bound by such SSJCA Codes of Conduct at all times..
The Australian Cricket Board notes. ”Players are recommended to wear a specifically designed properly fitting cricket helmet with faceguard. The Sutherland Shire Junior Cricket Association has legislated to make the wearing of helmets mandatory. It is also in the player’s best interest for the helmet to conform to the relevant Australian standard (currently AS/NZ S4499:1997 ‘Protective headgear for cricket’”). The provision of helmets is the responsibility of the individual player or, in the case of a junior player, his/her parents. The Club has a small number of helmets for Juniors that it will make available on request but in so doing does not accept responsibility for any incidents arising out of allegations concerning the fit of the helmet or any hygiene factor. It is recommended that players and parents having concerns regarding helmets should purchase such from a recognised supplier and ensure that the purchase meets Australian Standards.
