IEP Review 1302.61(c)(iv)

Policy

An IEP Review meeting will be held at the parent’s, Head Start Staff or LEA request or at the 1 year review date of the current IEP.

Procedure:

A. Remember that an IEP is good for one year and only needs to be

reviewed by the Multidisciplinary Team before that time if:

1. The child comes from another school district.

2. The parent requests a review for any reason.

3. The classroom teacher/parent feel the child has achieved the IEP goals

and new ones need to be set.

4. Classroom teacher/parent feels the child has regressed and goals need

to be revised or further evaluations need to be done.

5. The LEA requests a meeting.

B. When the LEA notifies you of a date for the Review you will:

  1. Send or take “Parent Invitation to IEP/Pre-referral Meeting” (KHS-504)and “Parents are Very Important to the IEP Process”(Before the Meeting)

2. The child’s parent should sign this form and return it to you.

3. This form is placed behind the “Parent Rights Summary Received” in the

child’s file.

4. If the date and time are not convenient for the parent, notify the LEA

and reschedule the meeting and notify the parent with another

“Parent Invitation to IEP/Pre-referral Meeting” (KHS-504)

5. Reference that you made this contact on the Family Contact

Record(KHS-800) and Document in Child Plus -Disability Section

(IEP/IFSP - Notes)and check this off on the Disability Checklist (KHS-501)

C. Take the child’s file and “Eligibility Criteria” with you to the IEP Meeting

1. All persons attending the meeting should sign the RMDT.

2. Look on the “Eligibility Criteria” and check the appropriate letter for

the child’s disability.

  1. If you are unsure what the child’s disability is, ask the specialist conducting the meeting which domain the disability would fall under. The specialist conducting the meeting should write recommendations on the back of the RMDT.

4. Remember that the LEA classifies all children under 8 years old as

Developmentally Delayed, (unless they choose to use categorical

then it will be only one area of delay and we will check that criteria letter).

We need this information for our Program Information Report (PIR).

5. Ask or tell the specialist writing the IEP of any behavior, transportation,

and medication issues that effect the child’s learning. If these issues

apply they should be addressed on the IEP.

6. Make sure the parents understand the proceedings and have the

opportunity to ask questions.

7. Make sure YOU understand the goals and services on the IEP

8. Ask the parent if they would like a copy of the IEP.

9. Send the Disability Coordinator a copy of the RMDT (KHS-506) and new

IEP. Keep one for the child’s file.

10. Put last years RMDT and IEP in the child’s file from last year.

11. Begin using the new IEP for your IEP Progress Report or SNW Activity Plan

as soon as you get it.

12.Reference this meeting on Family Contact Record (KHS-800) and

document in Child Plus -Disability Section (IEP/IFSP - Notes)and check it on theDisability Checklist(KHS-501).
