ICE TRAINING AGREEMENT – COLUMBIA CITY HIGH SCHOOL

This is a training agreement between the student-trainee, the training supervisor, the school and the parents of the student-trainee. Compliance with Federal, State and local laws governing employment of student-trainees will be required. All parties concerned acknowledge that the purpose and intent of the Training Agreement is to insure the educational value of the student’s cooperative training experience; and to establish an agreement with objectives to this end, that the participants are aware of, committed to and concerned with.

The Student-Trainee Agrees To:

  1. Comply with the policies and procedures of the training station in which he or she is employed.
  2. Abide by the rules and regulations of the ICE Program as outlined by the teacher-coordinator.
  3. Not to sever employment with the training station without approval of the coordinator.
  4. Record of work experiences will be completed and submitted as required by the school.
  5. Not to go to work at the training site any day when absent from school. (Dr.’s appointments, etc. should be scheduled during after work hours.) Anyone who works on a day when absent from school (without prior approval) will be assigned disciplinary measures.
  6. Terminate employment if he/she withdraws from school or the ICE program.
  7. If I am fired or released from my training station, I am no longer considered a full-time student at CCHS and will be expelled from school the remainder of the trimester thus losing all credits for that trimester.

Training Supervisor Agrees To:

  1. Work with the Teacher-Coordinator in planning varied training experience.
  2. Provide the student-trainee with at least 15 hours (3 hours per day) of work a week.
  3. Pay the student-trainee a starting wage of $ per hour and provide workmen’s compensation.
  4. Evaluate the student-trainee’s work performance every 6 week grading period.
  5. Contact the coordinator if termination of the student-trainee’s employment becomes necessary.
  6. Understand that on 2-hour delay and school closing days, it is parent’s discretion whether or not the ICE student-trainee will attend the training station.
  7. Personally attend or send a company representative to the Employer-Employee Appreciation Banquet.

Teacher-Coordinator Agrees to:

  1. Visit and observe the student-trainee on the job.
  2. Hear compliments and complaints regarding the student-trainee progress.
  3. Work with the training supervisor in developing training plans and in evaluating the student-trainer’s progress on the job.
  4. Instruct in the areas of communication skills, human relations, money management, consumer responsibilities and job search skills.
  5. Reinforce on-the-job experiences with related classroom instruction.

Parent/Guardian Agrees To:

  1. Communicate any concerns or questions with the ICE coordinator.
  2. Encourage student-trainee to maintain good work habits at school and on the job.
  3. Provide transportation to work or school if necessary for good attendance.
