liability concerning the bathing, drying, grooming, and/or consequences of the entire grooming service received by my dog. While the students at Cleveland Grooming Academy LLC.,practice the safest methods and techniques of grooming, the possibility of accidents is always present while working on live animals. If injury is to occur to my pet while being groomed I agree to pay all vet expenses, unless it is a result of gross neglect by the establishment.

I release and hold Cleveland Grooming Academy LLC., its employees and its students harmless against any and all liability damages, and/or expenses arising out of or in connection with pre-exhibiting conditions. Such conditions can include but are not limited to seizures, arthritis, collapsed trachea, back, leg, or disc pain, ear infections, and skin allergies. Sometimes the grooming process can expose a pre-existing condition; we cannot diagnose an animal, but may advise you to seek veterinarian attention.

I understand that by submitting this release form, the students and all agents of Cleveland Grooming Academy LLC., will always do what is best for the animal being worked on, which may involve the dog being shaved due to matting. Any dog that is matted and required to be shaved will have all owners notified and given the option to proceed with the groom, or to come pick up the animal.Cleveland Grooming Academy LLC., will not be held responsible for clipperburn and/or minor nicks resulting from grooming of matted, neglected coats or for irritation caused by removing coat from pets possessing mild to severe skin allergy.

I understand that the Cleveland Grooming Academy LLC., has a strict age restriction that only allows pets younger than 10 years to be practiced on by students. Grooming can be a stressful process for senior dogs, and should be placed with an experienced groomer. If Cleveland Grooming Academy LLC., is misinformed about a dog’s age, they are to be held harmless against and all liability, damages, and/or expenses that are a result of age related issues.
