1.This form is to be used for payment requests for individuals who are appointed by the University as external examiners of higher degree theses (MT1F and MT2F codes only).
2.Once authorised and signed, the form should be forwarded to Payroll Services (for examiners resident in Australia) or to Accounts Payable (for overseas examiners) for processing.
3.An examiner who is resident in Australia and/or who is subject to Australian income tax must complete an Australian Taxation Office (ATO) Employment Declaration Form and attach to
this claim form. If not submitted, or submitted without a Tax File Number, the ATO requires that tax instalments be deducted at the highest marginal rate. Payment (less tax) will be
made electronically to the examiner’s nominated financial institution. Payment Summaries
for Australian residents will be forwarded to the nominated mailing address at the end of the financial year.
4.Where the examiner is resident overseas and not subject to Australian income tax, income tax will not be deducted and full payment will be forwarded to the nominated mailing address. / Payroll Use Only
