HOW TO REGISTER WITH A NEW GP PRACTICE

To register with a new GP, you should take your medical card to a doctor of your choice and asked to be registered as a new patient.

Do not worry if you have lost, mislaid or never had a medical card. If the doctor can accept you, you will be asked to complete a New Patient Registration Form. You may also be asked to provide proof of residence in their practice area.

If accepted on to the practice list, they will then notify the Clinical Commissioning Group (CCG) of your registration with them.

How do I know which doctor to register with?

It is a good idea to ask friends and neighbours in your area about which doctors may be available.

If you have access to the Internet, details of NHS services are available on www.nhs.uk.

Your local Clinical Commissioning Group can advise you which doctors practice in your area. In addition, the local telephone directory includes a section on NHS practices and your local library will also have directories detailing NHS contractor services.

Services offered

All practices are required to have a leaflet available to tell you about the services they offer and this will enable you to compare them with other practices.

What do I do if I cannot find a GP who can accept me on to their list?

If you are ordinarily resident in the UK you are eligible for free treatment by an NHS General Practitioner. If you cannot find a GP to accept you on to their list, the Registration Department of your local CCG will help you. They will usually do this by speaking to your local practice on your behalf. If necessary, they will allocate you to a practice but this may not necessarily be the one of your choice.
