Do these things:

-Return to office request

-Forms that need attending signatures

-Confirm Pharmacy

-Urgent Studies/Test

-Superbill

General Process:

  1. Change to Ambulatory
  2. Indicate on front sheet and give to PFA any referral, lab, imaging, study, when you want these scheduled AND to return to clinic.

-If you order them after the patient leaves, follow up with Barbara to confirm and help schedule

Any orders entered into eclipsys will print to front desk, and the front desk will retrieve these papers from printer if they see on check out sheet that any test was ordered.

Return to office:

-Will be scheduled same day if needs within one month otherwise will be mailed appointment time

LP with IR (via email to Barbara) - order CBC/coags within 30 days, ask about ASA and anticoag!
+Athena request form if ordering Tau/AB / Social Work Consult (also do with AIM PMD appt if does not have PMD) order if you want HHA services
EMU / Return to Office

Prescriptions:

-Call Pharmacy if Allscripts is down

IF patient’s pharmacy is out of state. Can call but if it is for a controlled substance you can only write for 5 days. If you are calling technical failure, you have to email the state Bureau of Narcotics Enforcement at
