How to create Quizzes using Google Forms

Part 1: How do I get Google Forms as a Quiz?

1.Sign into your Google LCPS Drive Account.

There are two ways to open Google Forms and set up a Google Form.

2.First, Select NEW in the blue box on the left side.

1.A drop down menu will appear. Select More to see other Google Apps.

2.Select Google Forms from the More drop down menu.

3.This opens Google Forms.

4.Next, you will need to change the settings of Google Forms to set up the features of a Google Quiz.

  1. First, view the upper right hand corner and select the gear (Settings).
  1. This will pop up the Settings information and options. You may limit your students’ responses as well as the ability to edit after submission and a summary. Remember to save your changed Settings!

  1. Next, select the Quizzes Tab within the Settings.

  1. The Quizzes tab will allow you to change the settings and format the quiz.
  2. Click the sliding button to make the form a quiz.
  3. Select how you want the grade to be released: immediately or after some manual editing.
  4. Select what you want your students to see: Missed Questions, Correct Answers, and/or Point Values.
  5. Lastly, SAVE!

3.The Second way to open Google Forms into Google Quizzes:

1.Select the Waffle icon

2.select Forms from the drop down menu. (You may have to select “More” if you do not see the Forms icon.)

3.This will bring up all the Google Forms you have created and the templates for Google Forms.

4.Next, Select Blank Quiz to start your quiz! (Second option from the right.)

Part 2: Google Quizzes Functions

  1. First, Make yourself familiar with the Google Quiz Settings (the gear icon).
  1. The settings will allow you to:
  2. General: Restrict to LCPS users, Limit your answers to one Response, Edit after submissions, and more.
  1. Presentation: Have your quiz show a progress bar, shuffle question order, and your own Confirmation message.
  1. Quizzes: Make this a quiz, How to release the student’s grade, what the student can see.
  1. DON’T FORGET TO SAVE!
  1. Next, Title your Quiz and add a description.
  1. Select the “Untitled Question” to start entering in your Quiz Questions.
  1. Fill out your question and answer options. You may change your answer options by selecting the Gray Box and choosing from the drop down menu. (Tip: Make your first question or last question the student’s name. This way you have a record of who submitted which answers.)
  1. Next, select the Answer Key to take you to the grading function
  2. Select the correct answer and the point value for the question.
  3. Add Answer Feedback allows you to have feedback/information come up if the students get the answer incorrect or correct.
  4. Edit Question will take you back to editing the question and answer options.
  1. Don’t forget to make each question Required!
  1. The major features to any Google Forms or Google Quiz are:
  2. In Area 1:
  3. The Plus sign to add questions.
  4. The Tt to add titles within the Quiz.
  5. The Mountains image to add pictures or images.
  6. And the = sign to add another Section to your Quiz. (The sections allow you to separate the Quiz if you have multiple components to your Quiz. It will also jump the student to a new page and set of questions.)
  7. In Area 2:
  8. The two boxes image allows you to duplicate the question.
  9. In Area 3:
  10. The Trash Can allows you to delete the question from your quiz.
  1. Now, if you want to randomize the options so every student has a different order or go to another section based on the students’ answers:
  2. Select the three vertical dots
  3. Choose from Shuffle option order or Go to section based on answer.
  1. To view your completed form:
  2. Click on the eye icon in the upper right hand corner. This will show you how your form will look to your students.

Part 3: How do I get the link to this Quiz so my students can take it?

  1. Select the Send Box in the upper right hand corner.
  1. This will create a pop up where you can send the quiz to other people, add collaborators, and view the link to the form.
  1. Select the icon next to the envelope. This will take you to the link.
  2. Select Shorten URL to make it easier to use the link.
  3. Then click copy to copy the link to your quiz.
  1. This link can be added in Google Classroom or on any website you wish to use. In Google Classroom, make an Assignment and link the Google Quiz using the circled icon below.

Part 4: Grading

  1. After students submit their Quiz, they will be able to view their score by selecting the link: View your score. If you have short answer questions, they will not be graded until you manually go into each quiz to view the answers.
  1. To view Responses for your gradebook:
  2. Select Responses within your Quiz.
  1. This will take you to a summary of the quiz that shows the average of the class (Insights), which questions were missed the most (Frequently missed questions), and correct responses.
  1. To view individual quizzes:
  2. Select Individual. This will allow you to go through each of the students’ responses.
  1. To compile all the responses into a spreadsheet:
  2. Select the green box with the white plus sign. This will transfer your responses into a Google Sheet.
  1. Next, select a response destination: Create a new spreadsheet and select CREATE. This will put your responses in a Sheet in your Google Drive.
  1. To stop Accepting Responses from you students:
  2. Select the Accepting responses bar to turn it off.
  1. If you wish to Get email notifications for new responses, download responses or Print all responses:
  2. Select the vertical three dots next to the Google Sheets icon.
  1. Select from the options on the command list.
