Introduction

My Agency, the secure area of the Funded Agency Channel (FAC) website, provides registered users from funded organisations access to funding and payment information about their service agreement. If you require access to FAC and/or are not a registered user, please refer to the Accessing My Agency section of the FAC home page at:

Within My Agency, the Service Agreement Module (SAM) allows funded organisations to access their service agreement information and to update some records and documents through interactive modules. One of these modules provides organisation users with the ability to undertake the risk attestation online.

The following instructions detail the steps required to undertake and submit the risk management attestation form.

Using this guide

In this document we will show you:

  • how to find the Compliance sub tab to establish what action you need to take
  • how to complete and submit the Risk Attestation for your organisation

A red box will appear on each screen shot in the guide, to identify the area you need to select or action or note. In this example, the organisation name is being displayed for selection.

Completing your Risk Attestation

Navigate to the Service Agreement Module

  1. In My Agency, select the Service Agreement Module link

  1. A new window will open and the Service Agreement Module will the Current Agreements tab at the top of the screen will default.

Navigatefrom the Organisations tab to the Compliance sub tab

  1. On the Organisations tab screen, select your Organisation name hyperlink. The screen shot is an example only. Your organisation name will appear in the red box.
  1. Your organisation record will then be displayed.
  2. Scroll down the page to the sub tabs and select the Compliance sub tab.

Submit your Risk Management Attestation form

  1. In the Type column, find Risk Attestation. In the Status column it will appear as ‘Open’. In the Description column there will be a description of risk attestation.
  2. Select the row titledRisk Attestation to highlight the record.
  1. Then, in the top left hand corner select the ‘Open SACC Form’ button.

  1. The Risk Management Attestation form will display.
  2. Complete Questions 1 to 3 and if you have selected No or In Part to any question also complete Question 4.
  3. Select the Submit button to complete the Risk Attestation process.

  1. A screen thanking you for your completion (and submission) of the Risk Management Attestation form will appear.
  2. Select ‘Close’ on the screen.
  1. To see the ‘Status’ update from ‘Open’ to ‘Submitted’, select another tab (for example ‘Current Agreements’ or ‘Old Agreements’ or ‘Invoices’) and then return to/select the ‘Organisations’ tab. The new ‘Status’ will be displayed.
  1. Note: The Risk Management Attestationform can only be submitted once, annually.
