POSITION DESCRIPTION

POSITION:ADMISSION NURSE

REPORTS TO:ADMISSIONS MANAGER

Summary of Position: The Admission Nurse is directly responsible to the Admissions Manager for enrolling patients in the hospice programs and assisting with the referral/intake process. The Admission Nurse is committed to the philosophy, goals and objectives of State of the Heart Hospice.

Position Specifications:

Education:Graduation from an approved School of Nursing.

Licensure:Current registration in the State of Ohio and eligibility to obtain registration in Indiana.

Current registration in the State of Indiana and eligibility to obtain registration in Ohio.

Experience:Hospice nurse experience preferred. Minimum of two (2) years experience in Medical/Surgical, Oncology or community health nursing is preferred.

Special Skills and Requirements:

  1. Must have excellent literacy and writing, documentation, communication, and interpersonal abilities.
  2. Must have corrected hearing and vision within normal range.

3.Must be able to use tools and equipment required.

4.Shall have a valid driver’s license.

Working Conditions: Involves both office work and visits to patient homes. Travel may be in inclement weather. Must be able to lift 25 lbs. of equipment/supplies. Needs to be able to assist with patient care activities such as ambulation, transfers, and positioning. Requires varied amounts of sitting, standing, and turning.

Essential Functions:

1.Thoroughly and accurately explain hospice services to patients/families.

2.Establish and maintain appropriate records of patient care management in a timely manner following CMS and JCAHO guidelines and in accordance with State of the Heart policy.

3.Collaborate with the Admissions Managerto ensure referrals, intakes and admissions are completed accurately and timely.

4.Collaborate with Director of Marketing concerning referral resources.

5.Have a working knowledge of hospice benefits, in regards to Medicare, Medicaid and Private Insurances. Knows when to use Palliative Care Interventions’ and ABN’s and how to communicate benefits to patient and family.

6.In the absence of admissions and referrals, the admission nurse is responsible for the following:

  1. Routine Visits
  2. Continuous Care
  3. Chart Reviews
  4. Other duties as assigned

  1. Perform competent initial nursing assessments.
  1. Follow Universal Precautions technique.
  1. Performs other duties as assigned.
