Honor Rows Final Report Form

On your agency letterhead, please provide all information below within one month following your assigned game.

PROGRAM INFORMATION

  • Date of this report.
  • Name of person submitting this report, his/her title, and email address.
  • Title of your Honor Rows program.
  • Program start and completion dates.
  • Number of sites where program was offered and site locations.
  • Number of youth who firstenrolled in your Honor Rows program.
  • Number of youth who completedyour program (i.e., those who earned a seat to a game).
  • Number of youth who dropped out of your program andreasons why(e.g., violated pledge, lost interest, etc.).

FINAL REPORT NARRATIVE

Using the following order, please answer all questions in this section.

  1. Briefly outline your original Honor Rows goals and objectives, and briefly describe what participating youth had to do to earn an Honor Rows seat. Include key highlights about your Kick-Off event.
  1. How did you monitor and measure your program goals, including the anti-substance abuse pledge (i.e., what tools or methods did you use to track progress by participating youth)?
  1. Describe the population served (note demographics, such as race/ethnicity, gender, geographic location, etc.).
  1. How did adults (i.e., staff, parents, mentors) assist the youth in meeting their goals?
  1. In what tangible ways were the youth impacted by your Honor Rows program (e.g., grades, behavior, etc.), and how were they recognized for completing your program (e.g., via party, awards ceremony, etc.)?
  1. What obstacles did you encounter while implementing your program, and how did you address each one? Note any issues related to Honor Rows program operations, staffing challenges, volunteers, youth and/or parental involvement, and any others that directly impacted your program.
  1. Some agencies had a significant decrease (more than 15%) in overall youth involvement relative to the number of initial seats awarded prior to beginning their program. If your agency encountered this challenge, please explain what you think accounted for this attrition and what can be done to improve it next time.
  1. Briefly describe the game-day experience of both youth and adults.
  1. What did you learn that will assist you or others in serving disadvantaged youth and families?
  1. What improvements and/or changes do you want to implement in your Honor Rows program next season?
