Prospectus
  1. Purpose

To select in an open manner those athletes who meet the eligibility requirements for the 4th Hong Kong Games (HKG) as the representatives of Tuen MunDistrictCouncil (DC) to participate in the competitions of the 4thHKG.

  1. Introduction of the HKG

The HKG is a territory-wide major multi-sport event held biennially with the 18 DCs as the participating units. The objectives of the HKG are to provide districts with more opportunities for sports participation, exchanges and co-operation, to encourage members of the public to actively participate in sports, and to strengthen the public’s sense of belonging to the district they live in. The 4th HKG will be held from 27April to 2June 2013, featuring competitions in eight sports, namely athletics, badminton, basketball, futsal, swimming, table tennis, tennis and volleyball. Each DC will select in an open manner the talented athletes within its own district as its representatives to participate in the competitions of the 4th HKG. The divisions and quotas for the 4th HKG Futsal Competitionare as follows:

  1. Enrolment Dates

From 21 September 2012 to 22 October 2012

  1. Quota for the Selection Contest (The quota is 8 teams)

There are 8 teams in the men’s division with 5 to 18 players in each team. [If the number of enrolled teams exceeds the available quota, the sum of the total scores obtained by the 5 athletes with the highest marks in each team with respect to their registered divisions as endorsed by the Hong Kong Football Association will be used to determine the teams’ order of priority for participation in the selection contest. The divisions to which the players belong refer to the latest registered divisions as at the enrolment deadline for the selection contest held by the respective district. (The scoring method is: 4 marks for an athlete in the 1st Division, 3 marks for an athlete in the 2nd Division, 2 marks for an athlete in the 3rd/4th Division and 1 mark for an athlete in any division of the Youth League). If there are enrolled teams scoring the same marks, their order of priority for participation will be determined by drawing lots.]

  1. EligibilityRequirements

(1)Participants must be born in or before 1998 and be holders of valid Hong Kong Permanent Identity Cards or holders of valid Hong Kong Identity Cards who have resided in Hong Kong for 3 years or above. (Holders of Hong Kong Identity Cards must produce to the Organiser records of their first entry to Hong Kong, such as One-way Permits or passports. The date of their first entry to Hong Kong must be on or before 27 April 2010 to prove that they have resided in Hong Kong for 3 years or above.)

(2)Participants must reside within the district boundary of Tuen Mun District and be able to produce proof of place of residence.

(a)The district boundary of Tuen MunDistrict is defined in theElectoral Boundary Maps in respect of the District Council Election. For checking the district boundary of Tuen MunDistrict, please browse the website of the Electoral Affairs Commission:

(b)Participants shall submit a copy of a valid documentary proof of their place of residence bearing their names upon enrolment for verification of identity and record purposes by the Organiser. Failure to do so will render the application invalid. In addition, the Organiser has the right to request the participants to produce the original of the documentary proof for verification at any time, and participants shall raise no objection. If any participant fails to provide the originals of the relevant documents or the originals provided are different from the copies previously submitted, the Organiser has the right to disqualify him from the selection contest. Valid proof of place of residence includes:

(i)statement (such asmortgage statement, rates demand note, bill for electricity or water charges, monthly bank statement or telephone bill) issued by public utilities or commercial organisations or banks within the latest 3 months; or

(ii)declaration of place of residence issued by the Home Affairs Department; or

(iii)tenancy agreement or resident card; or

(iv)student handbook showing proof of place of residence (if a student handbook cannot be provided, the documents stated in (i) to (iii) above bearing the name(s) of the participant’s parent(s) together with the participant’s birth certificate may serve as proof).

(c)Student hostels and athlete hostels cannot be regarded as the place of residence.

(3)Those athletes who participated or will participate in the futsal competitions of the Olympic Games (excluding the Summer Youth Olympic Games and the qualifying round of the Olympic Games), the Asian Games, the East Asian Games, the National Gamesof the People’s Republic of China, the World Championships (excluding the World Youth Championship), the Asian Championships (excluding the Asian Youth Championship), the World Cup Qualifier, the AFC Asian Cup Qualification Round, the East Asian Football Championships or the Asian Indoor Gamesbetween 2010 and 2013 are not eligible to participate in the futsal competition of the HKG.

(4)Serving professional football athletes are not allowed to participate in thefutsal competition of the HKG, excluding the full-time athletes who are undertaking training and receiving subvention or subsidy offered by the government or relevant organisations (such as the Hong Kong Sports Institute, the Sports Federation & Olympic Committee of Hong Kong, China, and national sports associations).

(5)All athletes who meet the eligibility requirements for the HKG may form teams to participate in the Tuen Mun District Futsal Athlete Selection Contest.

(6)Each participant may only participate inthe selection contest of 1district. If a participant is found participating in the selection contests of more than 1 district, the whole team will be immediately disqualified with all its selection results cancelled.

(7)Participants must meet the eligibility requirements (1)to(6) mentioned above. If it is found that any participant fails to meet the eligibility requirements, furnishes false information, or represents more than 1 district or has participated in theselectioncontests of more than 1 district,the Organiser has the right to disqualify the participant and the whole team from representingTuen MunDistrictin the HKG.

(8)All information provided in the enrolment form must be true and correct and the form should be completed in detail. Allthe information entered (except residential address, contact telephone number and occupation) cannot be changed once the enrolment form is submitted. Participants should notify Tuen Mun District Leisure Services Office of Leisure and Cultural Services Department immediately of any change of residential address during the selection process, otherwise the participants will be regarded as furnishing false information. If a participant is no longer a resident of Tuen Mun District because of moving to another district during the selection contest period, he will be disqualified.

(9)A participant aged under 18 must provide a declaration signed by his parent or guardian before taking part in the athlete selection.

(10)The Organiser reserves the right to forbid any participant from taking part in the selection.

  1. Format

(1)Asingle knock-out systemwill be adopted.

(2)Each match will last 40 minutes, comprising 2 half-time periods of 20 minutes each. The half-time interval must not exceed 10 minutes. Matches are not played real-time (which means the clock does not stop when the ball goes out of play).

(3)All matches will be conducted in accordance with the Futsal Laws of the Game set by the Fédération Internationale de Football Association (FIFA).

(4)If a match ends in a tie at the end of the normal playing time, penalty kicks will be taken to determine the winner and the “sudden death” rule will be adopted.

  1. Method of Participation

(1)If anyone is fulfilled the eligibility requirement and Tuen Mun District residents and want to enrol in a Men’s division team, the duly completed “Enrolment Form” together withthe “Declaration” and a copy of the valid proof of place of residence(please refer to Paragraph 6(2)(b) herein) of each team member, to be kept by the Organiserfor record purposes,should be submittedin person or by a representative to the following office(s) within the enrolment dates stated in Paragraph 4 above.

(office hours: 8:30a.m. to 6:15 p.m. from Monday to Friday; closed on Saturdays, Sundays and General Holidays);

LCSD Sports Centres in Tuen Mun District (Tai Hing Sports Centre, Leung Tin Sports Centre, Yau Oi Sports Centre and The Jockey Club Tuen Mun Butterfly Bay Sports Centre) (office hours: 7:00 a.m. to 11:00p.m. from Monday to Sunday);

The original or a copy of the identity document(please refer to Paragraph 6(1) herein) of each team member must also be produced for verification of identity upon submission of the Enrolment Form.

(2)For enrolment by post, please send the duly completed “Enrolment Form” together withthe “Declaration”,and a copy of theidentity document(please refer to Paragraph 6(1) herein)and validproof of place of residence (please refer to Paragraph 6(2)(b) herein) of each team member to the TMDLSO (address: 3/F, Tuen Mun Government Offices Building, No. 1, Tuen Hi Road, Tuen Mun, N.T.) of the LCSD or other offices mentioned abovewithin the enrolment dates stated in Paragraph 4 above (the date of receipt of the form by the Organiser will be regarded as the receipt date) with ‘‘The 4th Hong Kong Games Tuen Mun District Futsal Athlete Selection Contest’’ clearly marked on the outside of the envelope.

  1. Announcement of the Shortlisted Teamsfor the Selection Contest

The names of the shortlisted athletes for the selection contest will be posted at Tuen Mun DLSO, Sports Centres of Tuen Mun District and Tuen Mun Sports Association (TMSA) or on the TMSA website on 26 October 2012 (Friday). The shortlisted teams will be notified in writing by the district selection board/committee/working group.If the shortlisted athletes do not receive the competition schedule before the competition, they may enquire the Tuen Mun DLSO for competition schedule on their own.

  1. Time of Team Leaders’ Meeting and Fixture Draw

The team leaders’ meeting and fixture draw will be held at 1800 hrs on 30 October 2012 (Tuesday) at 3A Conference Room, 3/F, Tuen Mun Government Offices Building, No. 1, Tuen Hi Road, Tuen Mun, N.T.. Representatives of teams are welcome to attend.

  1. Appeal

No appeal will be accepted by the Organiser. The decisions of the referees/umpires on the results of the contests and those of thedistrict selection board/committee/working groupon the selection results shall be final.

  1. Inclement Weather Arrangements

If a Pre-No.8 Special Announcement has been issued, or Tropical Cyclone Warning Signal No.8 or above or Black Rainstorm Warning Signal is still in force 2 hours before the reporting time for the first round of the matches on the contest day, all matches on that day will be cancelled. The Organiser will notify the participants of the corresponding arrangements separately in due course.

Notes:

Very High Air Pollution Level (i.e. Air Pollution Index: 101-200)

Activities shall be held as scheduled. The Environmental Protection Department advises participants with existing heart or respiratory illnesses (such as coronary heart disease and other cardiovascular diseases, asthma and chronic obstructive airways diseases including chronic bronchitis and emphysema) to reduce physical exertion and outdoor activities, to seek advice from a medical doctor before participating in sports activities and take more breaks during the activities. Children and the elderly are advised to reduce physical exertion and outdoor activities.

Severe Air Pollution Level (i.e. Air Pollution Index: 201-500)

Activities shall be held as scheduled. The Environmental Protection Department advises participants with existing heart or respiratory illnesses (such as coronary heart disease and other cardiovascular diseases, asthma and chronic obstructive airways diseases including chronic bronchitis and emphysema), children and the elderly to avoid physical exertion and outdoor activities. The general public is advised to reduce as far as possible physical exertion and outdoor activities, to seek advice from a medical doctor before participating in sports activities and take more breaks during the activities.

  1. Notes to Participants

(1)Each team shall prepare 2 sets of approved jerseys in different colours (all jerseys, shortsand stockings worn by the players shall be of the same style; one set in deep colour andanother in light colour) for the competition. If the colours of the jerseys are the same orsimilar, the second team named in the schedule of play (i.e. the visiting team) shallchange their jerseys or wear the set of jerseys in a different colour for the competition.The colour of the players’ jerseys shall be prominently different from that of the jerseysworn by the referees and the colour of the goalkeepers’ jerseys shall also be prominentlydifferent from that of the jerseys worn by other players.

(2)If there is any inadequacy in this prospectus, the organiser reserves the right to amend it at any time without further notice.

Tuen Mun District Futsal Athlete Selection Contest

Declaration

1.This is to confirm that all members of this team are residing in Tuen Mun District, and have never participated in any athlete selection contest of the 4th Hong Kong Games organised by other districts. If it is found not the case, the team will be disqualified.
2.All the information provided in the enrolment form by all team members is true and correct and the team members are eligible for the selection contest according to the rules set by the Organiser. The team understands that if false information is provided or if the information provided is not in accordance with the facts, the team will be immediately disqualified with all its results cancelled.
3.The team agrees to abideby all the selection results announced by the Organiser.
4.The team hereby declares that it has read the Prospectus carefully and agrees to comply with the requirements.
5.The team agrees to comply with the contest rules and abide by the decision of the referees/umpires. It also agrees to submit the identity documents (the original) of all team members for checking by the Organiser before playing in a match. The team understands that failure to comply with the contest rules will result in disqualification and no replacement of athletes will be allowed.
6.All team members are healthy and physically fitto participate in the above activity. The Organiser shall not be liable for any injury or death whichany participant may suffer in the activity if the cause of injury or death is due to the participant’s own negligence or inadequacy in health or fitness.
7.The team understands that if any participant(s) is/are found to be an imposter/impostors, the team will be disqualified with all its results cancelled.
