a) This form should be completed by the applicant and endorsed by his/her OC Unit.
b) This form should reach Officer Commanding Aviation Education Wing by email . Enquiries can be made to this email address also.

d) The information provided in this application may be shared with relevant officers / staff members in HKACC and the flying institute / organization as specified in Part 6 of this application for processing, record and/or verification purpose.

Solo standard as a student pilot on a powered aircraft, whereas the training follows a syllabus of Private Pilot Licence or above
Passenger carrying standard as a pilot on the Buckeye powered parachute
A current aircrew rating other than that of pilot
Another qualification of equivalent or superior standard of the above
An expired qualification of any of the above Note 2
