Make a copy for your records; return the completed form and all required documentation to Mrs. Naglik, Room 408. Incomplete applications will not be considered.Please be thorough by including all relevant information. All information will be used by the NJHS Faculty Council to assist with the consideration of your candidacy during the selection process. Completion of this form does not guarantee selection. Should you have questions about this form, please contact Mrs. Pille (Rm 416) or Mrs. Naglik (Rm 408). You must attach a final report card as well. Feel free to attach additional sheets if necessary.

II. Leadership Positions

Only those positions in which you were responsible for directing or motivating others should be included. III. Service Opportunities
List service activities in which you have participated. These can be individual or group service projects done either in or out of school. Generally speaking, service activities are those that are done for or on behalf of others for which no compensation (monetary or other) has been given (donations are excluded from consideration as service).
