High School ETMC-Quitman Auxiliary Scholarship Program

(Please Read Carefully)

SCHOLARSHIP PROGRAM

The ETMC-Quitman Auxiliary Scholarship Program provides financial assistance to current seniors in high school, volunteers, and employees who are seeking medically related undergraduate and graduate degrees.

These scholarships are awarded to students and employees who exemplify character through academic achievement,extracurricular participation, or excellence in job participation. In awarding these scholarships strong consideration is given to financial need. However, scholarships may be awarded to promising students/employees regardless of financial need.

SCHOLARSHIPS AVAILABLE

Students/employees who meet the eligibility and standards will be considered for an academic scholarship in the amount of $500--$1000. At the discretion of the Scholarship Committee, scholarships in lesser or greater amounts may also be awarded. The scholarships are intended as a grant to help pay for the cost of tuition, fees, and books. The scholarships are to be awarded and used in the same calendar year. Employees who are eligible may re-applyeach year. However, an employee who has been awarded an Auxiliary Scholarship two times is no longer eligible.

FUNDING

The scholarships are funded through donations and fundraising activities sponsored by the Auxiliary.

ELIGIBILITY

Students and Junior Volunteers graduating from high school in 2016, and employees from ETMC-Quitman must beattending or planning to attend a two or four year college, technical school, graduate school or post-graduate school. Applicants have to be entering the medicalprofessionor a profession associated toa medical profession.

REQUIRED CRITERIA

Students who will be considered for a scholarship must also meet the following required criteria:

Possess a cumulative 3.0 grade point average based on a 4.0 scale.

Provide copy of ACT or SAT score (only required for high school applicants entering college).

Complete the Application and Essay

Provide a current picture—this can be a copy, but must be a clear copy

Provide two letters of recommendation from individuals (no relatives), who can give an assessment of theapplicant’s past achievements and future potential.

  1. The first recommendation is to be submitted by any of the following: a counselor, pastor, teacher or professor. The recommendation must have been written no earlier than six (6) monthsprior to the ETMC-Quitman scholarship due date. The applicant must provide theperson who will be writing the recommendation on his/her behalf with a copy of the ETMC-Quitman Scholarship Recommendation Instruction Sheet and with a stamped envelopeaddressed to the Auxiliary Scholarship Chairman.
  1. The second recommendation is to be submitted byanETMC-Quitman AuxiliaryVolunteer (if you worked as either a junior or college shift volunteer) OR by an employer or a supervisor to an internship, a volunteer or paid work position who can attest to your work ethics. The applicant must also provide this person with the ETMC-Quitman Recommendation Form and with a stamped envelope addressed to the AuxiliaryScholarship Chairman.

Provide proof of residency in Wood County.

NOTE: It is the applicant’s responsibility to make sure that both recommendations are postmarked on or before04-17-17. Please be advised that we have guaranteed the confidentiality of the responses provided by your references and under no circumstances will the Auxiliary and/or the Scholarship Committee be

required to disclose this information to you.

APPLICATION PROCESS

Applicants must submit their handwritten completed scholarship application package to the Auxiliary Scholarship Chairman, Volunteer Services, ETMC-Quitman, 117 N. Winnsboro Street, Quitman, TX 75783 bythe postmarked deadline of 04-12-18 (date for high school only). Illegible applications or applications not containing all of therequired information and documentation will not be considered. The following documents are required of eachapplicant:

Completed Application Agreement (in packet)

Application

Official transcript showing a cumulative GPA of 3.0 or higher, converted to 4.0 system

ACT or SAT score

Legible handwritten essay

Current photo—or a clear copy of a recent photo

Two letters of recommendation (sent separately from this application)

Photo Release Form (included)

Copy of proof of residency in Wood County (tax receipt) attached to application

INTERVIEW

After the Scholarship Committee reviews the documentation from each applicant, you may be invited to interview before the Committee. Please note, if invited to interview, the interview is mandatory in order to be eligible for the scholarship.

SCHOLARSHIP FUND DISBURSEMENTS

If awarded a scholarship, the applicant will be notified by letter dated April 24, 2018. At that time the applicant will complete and send back the Response Form that accompanies the award letter. The Auxiliary Treasurer preparesscholarship checks payable to the school, college or university. The scholarship committee chairman is responsible forsending the scholarship check, along with a cover transmittal letter to the college or university financial aid orscholarship office.No funds will be given directly to students.
