INSTRUCTIONS: THIS APPLICATION IS FOR STUDENTS ENROLLED AT FAYETTEVILLE HIGH SCHOOL. Applicants must complete all items in each Section and return the form to Ms. Anne Butt by Friday, March 27, 2009. You must attach a current transcript covering the last seven academic semesters and a photograph of yourself. Two letters of recommendation are also required by the deadline and may be sent to Ms. Butt or Treva Hamilton under separate cover or attached to this application.
