HMO –UNITED HEALTH CARE – (100% OPTION)

a $20office visit co-pay. 100% coverage applies to in-network after deductibles and coinsurance.

Prescriptions are covered with a $10/$35/$60 co-pay.

Monthly Pre-Tax Employee Contribution

Employees are responsible for pre-tax contributions. Short-term Disability, Life and Accidental Death & Dismemberment Insurance as well as Dental and Vision can be purchased separately, (with life/STD disability as one package, dental another package, and vision a third package). Long-term Disability is a buy-in, buy-up option. The opportunity to purchase additional Life and Short-term disability is also available. Critical Illness coverage is available.

Your choices for insurance coverage are separated categories:

  1. The full package, which includes medical/Rx, dental, vision and individual life/STD disability, or
  2. Medical/Rx with life/STD disability, or
  3. Medical/Rx, or
  4. a. Stand alone dental, b. Stand alone Life/STD disability,or c. Stand alone vision, (or any combination of the stand alone products), with the employee paying the entire monthly premium for each (as listed).

Insurance is effective the first of the month following 30 days of employment.

The amounts are monthly payments and are a % of the actual monthly cost for insurance. DPM pays semi-monthly, therefore your pre-tax amount deducted each paycheck would be the amount listed divided by two.
