Eligibility: Benefits are available to regular full-time employees working in positions that require 40 hours or work per week after completing 90 days of employment.
Health Insurance Premium Reimbursement Program
Zumbro House, Inc. offers all full-time employees a health insurance premium reimbursement program. To be reimbursed an after-tax reimbursement up to $75 towards the employee’s monthly premium, employees must provide the finance department a copy of their receipt of payment within 14 days of the billing date by mail or via fax. Once payment has been confirmed, Zumbro House, Inc. will provide the employee with a reimbursement check or may reimburse through a payroll expense reimbursement, within 30 days following submission.
Once the eligibility requirements are met, employees can apply for coverage at any time.
Group Dental Plan
All full time regular full-time employees are eligible for our group dental insurance plan. Each eligible employee will have the option to join or waive participation. If the employee elects to waive coverage, he or she must sign a statement indicating as such. It is the employee’s responsibility to sign the declination form if not interested in coverage. The declination form may be rescinded at any time. The employee shall pay 50% of the premium for individual dental coverage – to be deducted on a pre-tax basis at each pay period - and the company shall pay 50% of the premium forindividual dental coverage. The employee is responsible for paying for 100% of the cost of dependent/family coverage. Note that coverage rates are reevaluated and adjusted by the insurance company on an annual basis. Current premium rates are available by contacting the human resources manager.
If an employee initially waives coverage and then decides to take coverage, he or she must wait until the open enrollment period. This is usually the month preceding the renewal month. Please contact the human resources manager for more information about open enrollment.
Eligibility
1.The employee is a full time exempt employee for dental and/or medical coverage.
2.The employee is eligible for coverage effective on the first day of the month following the 90th day of employment.
3.An employee is eligible for coverage on the first day of the month following the 90th day after advancement to full time exempt status.
Zumbro House, Inc. has the right to change insurance companies, change group plans, or to modify or terminate eligibility requirements, benefits, or coverage at any time and without notice.
401K Plan
Any employee who has worked 1,000 hours from their date of hire, is 18 years or older, and has been actively employed by Zumbro House, Inc. is eligible to participate in and ADP 401K retirement plan. Please see Human Resources for enrollment and plan details.
Paid Time Off (PTO)
The purpose of PTO is to afford eligible, regular full time employees the flexibility to use their time off as they wish. Regular full time employee is defined as any employee regularly scheduled to work 40 hours per week. PTO may be used at any time by an eligible full time employee for any reason including vacation, illness, doctor appointment, etc. PTO may be used only when the employee works fewer than 40 hours in a given work week, and should be used if available for any time off that is taken in accordance with the Zumbro House, Inc. Staff Reliability Policy. Non-exempt employees can use their accrued PTO in quarter-hour increments. Exempt employees can use PTO in 1hour increments. Up to 80 hours of PTO may be carried over from one calendar year to the next.
Zumbro House, Inc. requests a three week notice for scheduled time off. Requests should be made in writing on the designated “time off request form” and should be submitted to the employee’s supervisor no later than three weeks before the date of the scheduled time off. Time off is granted per the designated supervisor’s discretion.
New employees become eligible to accrue PTO after they have worked 90 days. Employees promoted into a full time position from an on-call position will accrue PTO after 90 days from the date of advancement. On-call/part-time employees are not entitled to PTO. Employees who are not entitled to PTO may request permission from their supervisor/manager to take up to one week of unpaid time off. An employee may not used PTO time if their time off is not in accordance with the Zumbro House, Inc. Staff Reliability policy.
PTO may not be used during a resignation or change in status notice period.
