Aberdeenshire Health Improvement Fund

The sole purpose of the fund is to support Health Improvement work and address inequalities in health across Aberdeenshire. The fund can be accessed by anyone who is working towards improving the health of the local community or communities of interest in a way that meets local needs and reflects local circumstances. The guidance aims to support applicants with their application and relevant funding application can be found at the end of this document. In addition further support can be provided via local Public Health Co-ordinator (contact details available at end of application).

If this is an Aberdeenshire-wide bid then please send to your local Public Health Co-ordinator. Aberdeenshire-wide bids will be assessed using the same criteria as local bids but in conjunction with all Public Health Co-ordinators to ensure Shire-wide consistency.

A scoring system will be used to ensure a fair and consistent assessment of bids across Aberdeenshire and that the evaluation process you choose will achieve proposed outcomes. Weighting will be used against key priorities see below:

·  Please note the Aberdeenshire Health Improvement Fund cannot be used to cover everyday running costs of projects for example, office rates or utility bills.

Guidance for completion of proposal form – 2014/2015

Health Improvement Fund spend should:

·  Reflect local priorities such as inequalities

·  Be evidence based

·  Focus on achieving measurable outcomes

·  Stimulate collaborative multi-disciplinary working

·  Add value to existing activity

Section 1 – Strategic Priorities

This document provides guidance on current priorities in Aberdeenshire to support outcome focused, evidence based Health Improvement activity in local areas. It is recommended that applicants review the guidance when planning their project.

·  Local Framework – Aberdeenshire Single Outcome Agreement

The Single Outcome Agreement is an agreement between Aberdeenshire Council, Community Planning Partnership and the Scottish Government which outlines how local and national priorities should be targeted.

To complete the Health Improvement Fund application processes please ensure that your project meets local and national priorities identified in the Single Outcome Agreement and related local indicators

Section 2 - About the Project

Within this section we are looking for detailed information on the following:

·  Anticipated Outcome – What will be the result of the project, what do you hope to

achieve?

·  Key Indicator – How will you measure the success of project?

·  Objectives – What do you need to do to in order to achieve your identified

anticipated outcome?

·  Have you identified the need for project?

·  Target Group – What is the group of interest e.g. young people

·  Number of anticipated participants – This can be an approximation

·  Timescale of project – Please detail the start and finish date for the project.

·  Partnership Working – Please detail all partners who will be involved in the

project and include details of who will manage the project.

Section 3 – Finance

This section is extremely important and detailed information is required. Only funding which has been identified in this section will be covered by the Health Improvement Fund. Any additional costs which arise throughout the duration of the project will need to be agreed by appropriate Public Health Co-ordinator prior to expense being incurred. Failure to do this may result in the Health Improvement Fund being unable to cover the full costs of the project.

The Health Improvement Fund is currently allocated on a yearly basis and all invoices will need to be sent to the Public Health Co-ordinator by the end of February, again failure to adhere to this deadline will result in project costs not being covered by the fund.

It is important to inform us if the project is being supported by any other funding source, this should include local funding sources and national funding sources such as the National Lottery.

Please note this funding must be accessed and used within the financial year.

Section 4 – Monitoring and Evaluation

Within this section we require detailed information on the monitoring and evaluation of the project. It is essential that projects are effectively monitored and evaluated. Two examples of planning and evaluation tools are available in the appendix. It is important to note that if there is no evidence of appropriate planning e.g. completed LEAP form, in initial funding application it will not be submitted to Health Improvement Group for funding. In completing this section information will be required on how individual projects will meet the SOA and anticipated outcomes identified in section 1 and section 2. In addition applicants will be required to provide information on how they will demonstrate success e.g. the evaluation method. Planning and evaluation tools such as those identified will support completion of this section.

Please include information on planning and evaluation tools to be used such as LEAP. Please note a plan for evaluation must be submitted before funding will be agreed.

LEAP

·  Leap stands for learning, evaluation and planning. It is an approach to planning and evaluation that is outcome focused, participatory and learning based.

·  The LEAP process is presented as a series of steps that respond to an identified need and develop into a continuous cycle of problem identification, planning, action, reflection, evaluation and learning. As part of the process the following questions are asked:

o  What is the need / background?

o  What needs to change?

o  How will we know we made a difference?

o  How will we do it?

§  What will we use?

§  What methods will we use?

§  In what ways will we use them?

o  How will we know we did it?

o  How useful was it / what will we need to do now?

·  Appendix 1 provides a template which can be used to help plan your project using LEAP process

WEAVER Triangle

·  This is a tool which can help to clarify the impact you want to make and separate aims, outcomes and activities

·  It is a method to visually present: what you are going to do; why you are doing it and what you hope to achieve by doing it

·  The WEAVER triangle is broken down into 3 sections:

o  Overall aim – this explains in very broad terms the change you want to see – which this project will achieve or help to achieve. Overall aims are very general to they need to be supported by more specific outcomes / aims

o  Outcomes / specific aims – Describe in more detail the results you want to achieve – why you are doing the project. These are the changes and effects that you hope will happen as a result of your work.

o  Outputs / Objectives – Describe what you are going to do in order to achieve your aims. They are the practical activities you will undertake

·  Appendix 2 provides a template which can be used to help plan your project using WEAVER Triangle process

End of Year Evaluation

You will also be required at the end of funding period to provide an evaluation report and for this reason it is essential that adequate evaluation planning and detail should be included in the outset.
