AAMC UNIFORM CLINICAL TRAINING AFFILIATION AGREEMENT

IMPLEMENTATION LETTER

The purpose of this letter is to provide a record of the clinical training affiliation agreement between the SCHOOL and the HOST AGENCY with respect to a clinical training experience for one of the SCHOOL’S students (identified below) and the agreement of the parties to abide by all terms and conditions of the AAMC Uniform Clinical Training Affiliation Agreement effective [June 4, 2015], which is hereby incorporated by reference, without modification or exception except as specified below.

Modifications or Exceptions:

1) The implementation letter pertains to undergraduate medical education student clinical and/or research training experiences.

2) Accommodations. Students who require accommodations in order to meet academic standards should contact the UCSF Office of Student Disability Services a minimum of two weeks prior to the start of any rotation or as soon as the need for the accommodation is identified. Later requests may necessitate rescheduling of the rotation until accommodations can be arranged. The SCHOOL will inform students who plan to rotate at UCSF of the need to request any needed accommodations as early as possible. The SCHOOL agrees that all expenses related to the accommodation of students with disabilities who rotate at UCSF under this agreement are the responsibility of the SCHOOL.

3) Term: Section D, Term and Termination, shall be deleted and replaced with the following: This AGREEMENT is effective on xxx 2016 as stated in the Implementation Letter and will continue through xxx 2021, unless terminated earlier. This AGREEMENT may be terminated at any time and for any reason by either party upon not less than ninety (90) days prior written notice to the other party. Should notice of termination be given under this Section, students already scheduled to train at HOST AGENCY will be permitted to complete any previously scheduled clinical assignment at HOST AGENCY.

This IMPLEMENTATION LETTER is effective as of [insert date]. The individuals executing this IMPLEMENTATION LETTER are authorized to sign on behalf of their institutions and certify that their institutions have accepted the terms of the Uniform Clinical Training Agreement and further agree to comply with its terms except as noted above.
