POLICY AND PROCEDURES

TITLE: Developmental Screening

POLICY

All children enrolled in Head Start and Early Head Start will be provided with a developmental screening witin 45 days of entry into the Head Start or Early Head Start Program. The screening is intended to identify those children who may have significant developmental concerns and may benefit from further evaluation.

PROCEDURE

During the first week of Early Head Start and Head Start the Family Helper will:

  • Check the Parent Permission to Participate Form to insure the parents have given consent for their child to be screened.
  • Children who are not screened due to the parents not signing the Parent Permission to Participate Form will be tracked on the DIAL/Denver Tracking Form and screened when/if parents consent at a later date.
  • Send out the DIAL 3 Parent Questionnaire for those children who will be taking the DIAL 3.
  • If parents do not return the Parent Questionnaire within two days, the Family Helper will contact them by phone to remind them to return it. If the parent/guardian has not returned the Parent Questionnaire within a week, the Family Helper will make one more attempt by phone to contact the parent to return the Parent Questionnaire.

Beginning the third week of school for EHS or HS the Family Helper will begin administering the Denver II and DIAL 3 to individual children.

When the Denver II or DIAL 3 is complete the Family Helper:

  • Enters the date the screeningwas administered, the results, and referral information into the PROMIS program.

NORMAL SCORE:

  • If the child scores within the normal range of development on either screening instrument the Family Helper sends a copy of the Denver II or DIAL 3 and a Screening Results Letter to the parents informing them of the results of the screening.
  • The original Denver II or DIAL 3, the DIAL 3 Parent Questionnaire (if applicable), and the Screening Result Letter will then be filed in the Child’s File at the HS Administration office.

SUSPECT or POTENTIAL DELAY

  • If the Denver II or DIAL 3 results show suspect or potential delay The Family Helper will send a copy of the Denver II or DIAL 3, a Screening Results Letter, the Permission to Refer and the Refusal of Services to the parents informing them of the results of the screening.
  • If the parents do not contact the Family Helper or return either the Permission to Refer or the Refusal of Services within one week the Family Helper will contact the parents by phone or in person and record their conversation regarding their child’s screening results on a Family Contact Note.
  • When the Family Helper receives either the Permission to Refer or the Refusal of Services they bring the Denver II or the DIAL 3, the Permission to Refer or the Refusal of Services and turn them in to The Education/Disabilities Coordinator to send to the referral agency or file.
  • The Education/Disabilities Coordinator will send a copy of the Denver II or DIAL 3, the Permission to Refer, Release of Confidential Information Form to the agency the parents have designated on the Permission to Refer Form and file the original forms in the Child Files.

Using the DIAL/Denver Tracking Form the Family Helper will track those children who do not get referred due to the parents signing a Refusal of Services or whom the Family Helper was unable to contact and rescreened later in the school year.
