Dear <Name>

I am sorry to hear from <manager’s name> that you are currently unwell and therefore unable to attend work in your role as <post> with Hampshire County Council.

<Manager’s name> has asked me to contact you as s/heis becoming increasingly concerned about your continued absence and because you have not been in touch to advise her/himwhen you intend to return to work.

I understand that <manager’s name> wrote to you on <date of LM’s letter> asking you to provide her/him with a Statement of Fitness for Work to cover your absence since <date statement expired>. <Manager’s name> also asked you to make contact with her/him to discuss your intention regarding a return to work. I understand that you have not made contact with <manager’s name> and have not provided her/himwith the Statement of Fitness for Work as requested.

Having considered the circumstances, it is my view that your continued absence is unauthorised and I have therefore no alternative but to suspend your pay with effect from <date> when your last Statement of Fitness for Work expired.

Please contact me urgently on <HR Telephone Number>or by email <HR Email> or in writing, to explain the reasons for your continued absence from work. If this is not possible, please arrange for someone else to contact me on your behalf.

As you will be aware from <manager’s name> letter dated <date of LM’s letter>, it is a condition of your service that all absence over seven days is supported by a Statement of Fitness for Work. Please would you sendthis to me to cover your continued absence as soon as possible. A pre-paid envelope is enclosed.

If I do not hear from you by <date - 10 days of the date of this letterI will consider what further action will need to be taken.

I have forwarded a copy of this letter to your home address and to your emergency contact address in an effort to make sure all reasonable steps have been taken to contact you. I look forward to hearing from you.
