Legacy award –Presented to an HBA Chicago chapter volunteer who has consistently demonstrated outstanding service to the chapter over 5+ years.

Please include:

  • years of service
  • committee(s)/projects with which the nominee was involved
  • leadership or other role(s) held in those committees or projects
  • five key accomplishments that best highlight the nominee’s “outstanding service” to the HBA Chicago chapter

Mission Impossible award –Presented to an HBA Chicago chapter volunteer who(or volunteer team that) embraced and executed a difficult challenge leading to the overall betterment of the HBA Chicago chapter

Please include:

  • short description of the challenge and why it was difficult
  • how the challenge was tackled and the success executed
  • what was the “betterment” seen by the HBA Chicago chapter (even soft metrics would be helpful)
